I think Ed basically works for the manufacturers and is paid to say what they want said. They are for whatever sells the most machines and whatever makes their customers (the DMEs) happy. They work for their customers, the DMEs. My views align with theirs only in the sense that I would like as many machines as possible made and put in the hands of all people who need better sleep.
That said, the manufacturer-DME cartel members are not the best people for deciding what is best for patients beyond making machine available. Medical people and patients themselves, in partnership, are best at truly medical decisions, even as messed up as the entire industry-cluster is at this point, medically speaking. (Although I am not personally a medical person. Or impersonally a medical person, for that matter.)
So my position is that CPAP should be available to anyone who might get better sleep using one, as decided subjectively by the patient based on response to pressurized air. However, anyone continuing to have sleep problems surely needs full involvement of a doc and his/her staff. Not full involvement of a crutch-salesman business and their staff.
Put the DMEs and HST people out of business. Stock the shelves of Wal-Mart with inexpensive, quality CPAP machines. Let response to treatment be the measure of medical necessity. Educate the doctors about sleep disorders, and let sleep labs get involved with the people who can't sleep better with some simple pressure from an OTC machine.
If the public were ever to become the customers of the manufacturers, instead of the DMEs being the true customers of the manufacturers, market forces and medical services (as opposed to DMEs) ought to be able to take it from there.
Until that happens, Ed is stuck working for the ASAA, the ASAA is stuck working for the manufacturers, and the manufacturers are stuck working for their customers the DMEs, who presently head the industry in partnership with the insurance people in the big ugly cartel.
In the meantime, until that mess gets sorted out, I'm glad we have cpaptalk.com, which so far for the present the CPAP cartel has been unable to shut down.
That said, let's all watch one another's backs, shall we?
