Thank you for all of your replies!! I can't even begin to tell you how much this has helped!

My Sleep Doc is not a Fan of APAP. He feels that the machines do not respond quickly enough to an event to make the therapy work as well as it can.Sheriff Buford wrote:Here we go (lookin' at Pugsy )... I'm only referring to the Resmed S9 Autoset (not Philips Respronics). To me its a comfort thang'. My theory (as been discussed here before, with little objection) is that the Resmed autoset will not start increasing pressure to treat an apnea event until 10 seconds. This allows for yawns, coughs, temporary events that folks hold their breath, etc... Once the machine starts to "ramp-up" during an apnea event, it may take 4-7 seconds to achieve the needed pressure to treat the apnea event. This means your body went thru an apnea event for 11-17 seconds before being treated. My theory is that I didn't feel as well as if the machine was blowing a set rate of air (cpap mode). I've done enough tinkering to accept this for me. Whatever mode you use, I strongly suggest folks to switch to the alternate mode once or twice a year to see how you feel. Everyone is different and the 11-17 second delay the autoset causes does not make a difference in how they feel. To each his own!!
A few months back, I tried the autoset mode and felt better. I stayed on the autoset mode for 3 -4 months. Last week I went back to cpap mode, and didn't feel any difference. I'm still on the cpap mode, but... I seem to like a little more air that the cpap mode offers when I first lie down. If you can see a big difference on how you feel, then by all means stick to the mode that makes you feel better. Most of the time, I can't see a big difference, but that's just me.

Nonetheless, I stil got an APAP machine so I can do an occasional "at home" self-titration just to see what may be changing in my pressure needs.
I run in CPAP mode at his direction. It took some getting used to when I switched from APAP to CPAP, but now I'm fine with it. I guess I could run it in APAP mode with my pressure set as the minimum, but since I feel OK and my AHI's have held pretty steady at 1-1.5 per night, I guess I good to just leave it alone..... if it's working, why mess with it ?
I don't know how many other Sleep Specialists feel the way mine does, but I guess he has a valid point, or at least has convinced himself tha using CPAP only is in the best interst of his patient (me).

Three months ago my apnea's went up a lot. It felt like I wasn't getting enough air. ( I wasn't all of a sudden sleeping on my back ) Called the VA clinic and they set my machine to APAP 14-16. It had been set at 14 CPAP. It took about a month and my apnea's returned to the low numbers that I was used to. If during that month I hadn't tried a new mask for a week, it would have worked faster.

APAP...
I started out on APAP 4-20cm while waiting for my titration study. I self-titrated with help from this board and had my minimum pressure up to 9.6cm before my titration study. My sleep doc wrote my titration Rx for straight CPAP 10cm, but my study noted a max of 12cm. So, I adjusted my min pressure to 10cm, but left my S9 in APAP mode and tightened the range.
Based on my sleep data I have determined that my needs increase to 11-12cm in REM sleep. That is why is stay in APAP mode.

I didn't choose. The doctor prescribed APAP.
