1st night as a hoser!

Post by mboy » Fri Oct 09, 2009 4:21 pm

Here is my sleep study I posted earlier in week:

http://funkymunkys.com/sleepstudy1.pdf

Had my 1st night last night and a 2 hour nap with my new Auto M.

Not too bad I think.

AHI for the total 9.5 hours on the auto M is 5.7 (that has to be REAL good for 1st night considering where I came from).

90% reading was 16.5cm and my Leak was 66ish which I think is very very high (not sure), but there was a time or 2 I took the mask off while machine was on (for a minute or so), then put mask back on.

Not many issues. felt a couple of tiny leaks here or there, but managed to adjust on the fly with no issues (fulllife).

I do notice on the fullife that their is a constant stream of air coming out of the front of thae mask. 2 little holes where hose connects. I imagine that is normal?

I did wake up a couple of times during night, but fell right back to sleep.

All and all, not a horrible experience. DEF. something I was already committed to going full force on and 1st night has not changed my plan at all.

Thoughts on my 1st night?

Look in your mask literature for the intentional vent/flow rate for the pressure that you are using.
Your 66ish leak may not be as bad as you think. With higher pressures we expect more flow rate.
Remember the Respironics machine shows the whole thing so you have to subtract the flow rate from the rate shown to get real leak or leak average. Remember also that if you take mask off and machine blows air, it will impact your leak data average.

All in all good job for a first night. Many do well to keep the mask on and sleep 2 hours. I think you did very, very well.

Thanks.

is my AHI of 5.7 decent?

Should I be striving for lower.
My sleep study AHI was in the mid 60's so I HAVE to think 5.7 is a step in right direction.

Yes, the AHI of 5.7 is very decent for a first night.
Give things a bit of time. See if the leak changes any. If after a week or two you don't see any drop in the AHI then maybe, just maybe, some little tweaking. Way too soon to tell right now.

Let's see how you feel. Some people feel great with 5-8 AHI, some people feel like crap. Some people need a bit lower AHI to notice improvement in how they feel. I am one of those. I need less than 3 AHI. You will find what works best for you but you have to give it time.

I forget, did you get the software? Or are you using the LCD screen? Do you know how to reset those averages to zero so that you get nightly numbers instead of averages?

I have the software on the way, already have card reader. The results I posted were from LCD as software hasn't arrived yet.

How would one go about lowering their AHI? HIgher CM?

Ahh, decrease leak if at all possible, then consider slight increase in pressure.
But don't be premature with thinking you need to increase pressure immediately. One night of numbers is just one night. You will find that they vary from night to night, especially at first.

Give it a little time and wait and see what the software shows. Then you will understand things much more.

A number is just that, a number. But when you get your software, you can see trends. The leakage number can either be a constant thing thru the night, but it can also involve "spiking" around apneas. Getting the software for me was a real eye opener (especially with folks here to help me interpret it.). Also important to see if there are trends to when your apneas occur, and what the pressure is when they occur.

2nd nigh last night.

Woke up a few times with some leaks, so stillhave some adjusting to do with the mask. I need to get the quick release clips for sure.

Anyway, IU did not have to get up earely this am, so got a good amount of sleep last night, even with a few wakeups.

My AHI this AM was down to 4.7 from 5.7 my 1st night (from non reset 7 day avg screen).

90%CM was 17.5 up from 16.5, so my guess is I need a pretty high pressure on avg (which I knew).

Pugsy, can you tell me how to rest the avg so I can see what it is each night while I wait for my software?
Does anyone know how to program the start date screen?

Thanks again.

I def feel less groggy this am then I typically do.

I did not take the mask off at all last night until I got out of bed around 9am.

Congratulation, mboy. You seem to be doing fine!

90% is actually the 90th percentile. It means your machine went above that pressure only 10% of the night. The rest of the nigh was spent at 17.5 or below it.

To reset your averages to zero each night on the M series machine. You will be in the clinician menu when you do this. Be aware that once you do this you may see zero the next morning til the machine decides to store the numbers. When I did it sometimes I got the "new" averages fairly early and sometimes it was the afternoon before they showed up. So don't panic if you don't see the new numbers right away.

==========Resetting LCD Display Data====================

To reset LCD Therapy data (does NOT erase any data from SmartCard):

1. Hold down the (<-) (->) buttons while plugging in the power on the back,
wait for 2 beeps or 5 seconds, release buttons.
2. Press the (->) key and navigate to Therapy screen.
3. Press and HOLD down the (-) minus key for 5 seconds and/or until therapy data is reset to Zero.
4. Press the (->) key to scroll back to main menu, press (+) key to enter Setup or
On/Off button to exit.

Not sure what you mean by start date? What I did before we got the software was reset the machine each day and keep a log of daily data and some brief notes about any problems and/or adjustments made. Hope that helps.

I understand that dates are determined by the encore software once the card has been read and reinserted in the machine - it will get local time and date off that first PC the card is read by at least in encore pro - assume similar thing is done in viewer
