My CPAP machine wakes me up with pulsating air

Hi All!
It's been a while since I've posted, but I am really struggling lately. But my therapist says I'm within OK range, though I don't feel ok, very aggravating. It seems like there is a surge of pulsating air when I have some apnea event. I woke up this morning to what seemed like an aggressive series of pulsating burst of air. I've also been having a lot of trouble sleep generally and waking in the middle of the night, for no apparent reason, like peeing or noise and I don't notice the pulsating in these frequent awakening.
I mainly want to see if there is something of interest with the 2 wake ups, the first one I didn't notice the pulsating, just woke up with no stimulus that I remember. The second wakeup was pulsating.
Does this, the pulsating air, sound like a familiar problem?

Speculate? Perhaps FOT gone awry (although FOT is 4 Hz).

In the past this was dismissed as cardioballistic artifact (heart pulses sent through to the transducer), at least until a known bradycardic with a heart rate of 40 was having 80 of these pulses a minute.

I'd give ResMed tech support a call, but I think we might be looking at a recall.

If it's exactly double the heartrate, could it be cardioballistic near the lungs plus blood vessels near the upper airway giving a doubled artifact?

I don't follow what this all means. Are you both thinking this is a false reading from my CPAP machine?

Sorry. Rubicon is an expert at reading the charts and is noticing some oddness in a few people's charts, so I was throwing out to him a possibility for how the charts look. He is figuring out, I believe, whether ResMed may have a sensor problem of sorts in the making.

The FOT that Rubicon mentioned is a forced oscillation technique some PAP machines use to assess the state of the user's airway.

I am most decidedly NOT an expert at reading charts.

But what you describe can sometimes be a sensation the machine gives when it is trying to discern whether a pause in breathing is accompanied by an open airway or a closed airway and the machine uses FOT to do that. It can feel to some like a vibration. If that is the sensation you are describing, it is a feature, not a bug, and can be ignored.

The actual reasons for your waking up are something to be explored, of course. But if you are also having an apnea at that time, it is natural for the machine to cause the slight vibration. And if your pressure feels high to you at the time, that may be more the irritation to you than the actual pulsing.

What I've said a million times (OK maybe 4) is that you don't know it's CBA unless you have a synced EKG. Everybody who's coming in has those pulses at ~1.2 Hz (we gotta talk Hz cause FOT is measured in Hz so when we (you guys) call up ResMed and say they're pulsations and they say Oh it's FOT you can respond No it's not cause this is 1.2 Hz and FOT is 4 Hz and besides FOT doesn't kick in till about 8 seconds into expiration and this starts immediately).

Now that we have a wake guy saying WTF! pulsations! I think this lends credibility that it's machine fault. My working theory is that it's having trouble maintaining baseline. Here in Rizzo's waveform magnified like crazy, pressure should be 8.26 but it's actually wandering all over the place.

OK, chicken-egg? Is the patient causing that or the machine? Can't tell using our simple tools, but in these several cases we have up now I think there's enough info to submit to ResMed. It occurs often enough that a bunch of people should have complained already and they should come up with a response that satisfies everyone.

If not, then what you do is send in the data to FDA. I can assure you, THEN you'll see some activity.

Since I'm not a talented chart reader, I see nothing exceptional in your images, myself.

Sometimes when I wake up, I feel the vibrations of the FOT, myself. Other times not.

Your running in APAP mode might allow the pressure to prevent events in REM toward the end of the night. Maybe. Just my opinion.

Preventing awakenings in the middle of the night can sometimes be about sleep hygiene principles, so here is a link on that:
