Sleep help

Post by morpheustakethewheel » Wed Aug 07, 2019 7:12 am

I've been on the CPAP for over a year. Right now sleep is bad and I feel especially exhausted - this happens occasionally and a med change or something can take me from "miserable" to "tolerable" but never actually "feeling well-rested and good."

I'm running late for work and a little anxious that there will be something inadequate in my data and I'll have to wait all day to get home and fix it but .. here goes. Thanks in advance for any help.

When you get a chance we also need the stuff on the left side....in this format along with the flow limitation graph if you are using a ResMed machine.

Any special reason for the extremely tight auto adjusting range?

Waking often? Trouble going to sleep or staying asleep?

What meds are you taking? Even OTC.

Took a quick look but didn't see how to make that left-side data show up, will try again tonight.

Auto-adjusting range - I'm not sure, I'll have to figure out what you mean and what the factors might be.

I wake often. I don't usually have trouble going to sleep, but staying asleep is hard. I wake up with pain in my hips/low back, twitchy legs, hot flashes or for no apparent reason at all. The hot flashes are subtle at this point but pretty well controlled by turning down the thermostat and using meds.

Meds: Pramiprexol (for RLS), melatonin and clonidine (for hot flashes). Sometimes tylenol.

I don't know why you aren't seeing the left side stuff...but if you can't figure it out create a new profile and import again...and it should be there. It's the default.

Your machine wants to go higher but can't. I suspect flow limitations are going on but can't see them to get an idea just how bad they are.

Are you experiencing any nasal congestion? Sometimes nasal congestion will fool the machine into thinking it needs to kill some FLs.

The fragmented sleep itself from whatever reason is going to make you feel like crap during the day.

You've got some sleep maintenance insomnia going on and it might not all be related to OSA.

Your Pramiprexol isn't helping things either...one of the side effects is trouble sleeping. Caught between a rock and a hard place with it I understand. Damned if you do and damned if you don't.
The clonidine also can cause trouble sleeping...known side effect.

People often assume that just because a medication comes with a "may cause drowsiness" sticker it should automatically help with sleep and that simply is not the case. I have that same problem myself with a pain medication that I need for severe arthritis.

You mention hot flashes....are you female and going through menopause?

I think I've got the left side sorted out (View > Daily Sidebar) - the f8 clue helped me with that.

I do have occasional congestion and just resumed allegra (morning) and nasacort (evening).

I am a woman. I am not currently in menopause or even perimenopause, but I'm over 40 and occasional hot flashes are now a thing.

Can I see the flow limitation graph please and tell me if you had much nasal congestion.?

I had some congestion last night, but it was mild.

I have seen worse and I have seen better.

To give you a reference point as to things....this is mine from a few nights ago.

I don't know how much of a factor your FLs are in terms of your poor sleep quality. They could be playing a part though and for that reason it might be worth considering increasing the maximum a bit and letting the machine try to kill those FLs. It wants to but it can't do much with your tight range.
I am not sure if this is the correct video but I think it talks about how FLs impact us.

Was there a special reason for the tight range....aerophagia issues perhaps (air in the belly) or something?

Are you okay with making minor changes to your settings or do you prefer your medical team to do it?

To my knowledge there's no reason for the range.

As long as I can figure out adjusting the settings, I'm comfortable doing it. It's not immediately obvious but I'll see what I can figure out.

The manual explain the secret handshake so you can get into the clinical setup menu and make changes.

https://www.respshop.com/manuals/ResMed ... %20her.pdf

Hard to say where to put the max but maybe around 12 and lets see if the machine wants to go that high.
Don't want to make too drastic of a change because your sleep quality is already so fragile.

Here's the report from last night. I don't recall waking up in the night and I think I feel less exhausted - I'lll see how I feel throughout the day.

Thanks for all the help with this.

I definitely felt better yesterday - not well-rested and energetic, just not miserable. Which I would expect even if all my settings were perfect since sleep deficit recovery doesn't happen in one night.

The pressure looks better at 12, there's one bit early on that looks like it wanted to go higher - maybe from 12 to 13? The flow limit is still hairy. Poking around the forum I'm seeing recommendations to address this by raising the minimum pressure.

Thoughts/recommendations welcome.

Min 9, max 20 on the pressures.

All limiting the max does is prevent the machine from being able to respond...

Raising the min pressure enough is what is key to getting the best results.
