Just wanted to Introduce myself and post my starting settings and let you guys know about my situation.

34 Years old.Male. Athletic and in excellent health. My name is Tyler and I live in Saskatoon,Canada

Was Diagnosed with OSA with an AHI of around 20. ( Was lower when side sleeping)

My only real symptom which I found and was going to the doctor before was fatigue. It was pretty consistent and I never woke up feeling refreshed. I did a trial run with the equipment listed below and decided to purchase online instead of from the company I tested with. ( Im in Canada and the machine is covered 90% of value but you have like 3-6 month wait to get the machine and it isn't an auto machine.)

I decided to purchase online on my own dime and buy the auto machine and equipment I did the week trial with. The drawback of this is that now I have to set the settings myself until I can get into to see a specialist which is probably a year wait. The first night I left the settings on factory settings and didn't sleep the greatest timewise but my AHI as read on machine was 2.9. The next night after guessing half in the dark for some settings to try I slept around 8 hours but had a AHI of 9 with a leak rate of 4L. ( I tried to loosen the nasal pillow as it has been bruising nose bridge but I'm not sure if this is reason or maybe mouth opened?)

So I did some reading on here from forum posts and decided to try these setting and I would like to know what you guys think?

Any questions or concerns please ask as this is something I am taking very serious.

Settings are:

Autoset: 6-20 Nasal
Response:Standard
EPR: On, Full Time Level :1
Climate Control: Manual 80 Degrees Humidity Setting:4

I will purchase an SD card and take a weeks data before posting here.

A year wait ? That's criminal!

Welcome to the forum. I am sure the experts will be by soon to help you. To me, that sounds like a good starting point but if you feel that you are suffocating you might want to bump the minimum pressure a little. :)

I should mention as well the changes I made from factory setting the night my AHI was 9 was to change pressure to 6.6-16 and to turn on EPR to 3 Fulltime.

You ought to have things all smoothed out long before you get to that specialist.

So had another night using the machine settings were 6-20 Pressure with EPR turned off. I had AHI of 2.1 and feel not too bad but I did have 18l leak rate and woke up more than once with mouth super dry and had a weird feeling like my cheeks bubbling.

What do you guys recommend??

Stick it out a few days and adjust to pressure ?

Congratulations on starting apnea treatment! Sounds like you’re having leakage through your mouth. Try suctioning your tongue up against the roof of your mouth and see if that helps. People will be able to give you lots more advice about that and your settings once you get that SD card and post a chart.

i say stick it and let your brain and body adjust to the pressure.

also, make sure you get that sd card sooner than later so you can post charts.

Should have SD card by tuesday thanks to Amazon Prime! Does the machine keep the data on it without a SD card or do you need to have the SD card in it to get that extra data I see you guys posting?

The SD card needs to be in the machine - only summary data is stored otherwise.

Your lower pressure of 6 is a little too low. I would suggest you try it at 8 and see how it goes. If 8 feels like too much of a jump, try raising it a little bit each night until you get to 8. You may find that a 2 cm jump is not very much. Some people are very sensitive to pressure increases and others aren't. Personally, I never really noticed any difference when I raised my pressure by 2 cm.

Thanks I've turned it up to 6.6 and will increase over the next few nights to see results. I had definite mouth leaks so I'm going to try mouth tape and see what happens as well tonight.

I would also like to know if you guys can tell that the machine is actually treating apneas and that I actually do have OSA happening. Any other advice based on my first results is welcomed and appreciated.

It's obvious your machine is reacting to something. If you look at your pressure graph, it keeps going up in response to something, and then goes back down for a short period of time, then repeats. Something is causing the pressure to go up. If you were not having events, the pressure would stay steady. It is possible your machine is reacting to flow limits and trying to prevent an event from happening. Even with the machine, you had seven hypopneas and one obstructive apnea during the night. These events happened in spite of the machine trying to prevent them.

If your machine is reading correctly despite the leaks, then the sleep apneas that are getting taken care of by the machine are roughly the difference between the number you saw on your sleep test, and the reading of the machine for the night. The apneas that you see on your machine are obviously only the ones that your cpap couldn't handle. Although the leaks look much better on the second set of sleep data than the first *smile*. Hopefully that's a trend, and the leaks stay as good as the second set of sleep data you posted. Going from about ~5% in large leaks to less than .5% is a really good improvement.
