The question being asked is not at all clear - are you asking how AHI is calculated, or are you asking how the “correct” pressure for therapy is determined?

AHI is a very straightforward calculation - number of total obstructive apneas and hypopnea events divided by the amount of time involved.

Treatment pressure is, as Pugsy so clearly stated, a matter of trial and error - you adjust the pressure upward until the number of events is minimized, while keeping an eye out for side effects or CAs.

Perhaps you could rephrase your question to make it easier for everyone to understand exactly what information you are looking for.

Since My AHI is 136, without the APAP &/0r VPAP, With it My AHI is 1 - 2, forgot the Exact #.
I had 136 Apneas Per Hour!! Before APAP.


I am surprised that My Pressures weren’t 15cmH2O — 20cmH2O, not 4cmH2O — 8cmH2O?!?!?
How is the “Correct” Pressure for Therapy determined?!?

There is no direct correlation between sleep study AHI and the pressure needed for the best therapy. Also, the sleep study may not directly correspond to the AHI you would have in your own bed. For example, my AHI was 82 sleeping on my back, but only 24 on my side. A pressure of about 10 eliminates events if I stay on my side; I'm guessing it would have to be about 15 on my back - fortunately I learned to stay on my side!

Actually the problem was worse than simply the basic numbers: when I slept on my back I had clusters of 20 or more events in a few minutes, driving my O2 levels dangerously low. This was a more serious matter which led me to spending a lot of time staring at SleepyHead (OSCAR) charts and trying to fully understand my sleep patterns.

As I started: 2017 August 16Th, I was Started on APAP, with the Pressure: 8.0cmH2O, had Ramp Available but, I couldn’t tell if It was Set By: DME Supplier, than on 2018 Mar 16Th, after Mechanical Problems W/ 1ST Machine! The DME Delivered My Current VPAP III,W / Pressures: 4.0cmH2O — 8cmH2O — RAMP 5Mins — Set By: DME, which I still have.
The ???? Is?
What determines when You need a APAP or VPAP?

My Respironics REMstarAUTO | M -Series W / A - Flex, did Better @ Home in My Own Bed! His numbers were 1 — 1.5 AHI!

The Philip -Respironics System One OmniLab Advanced ➕ Titration System @ the Sleep Lab numbers were 1.5 — 2 AHI
