Does my Respironics catch OA and H?

Post by SleepySwiss » Tue Feb 26, 2019 4:35 pm

<r>I have a Respironics machine with humidifier since about 2 years. I have a heated hose and use a F&P Eson nasal mask. I love the machine and have no problem with breathing etc. However, at anything above an AHI of 2 I do not feel well rested, below 1.5 I feel 'fully functional.' The first year+ I was fluctuating a lot between ~2 and just below 5. I then started to take Ashwaganda and I dropped to 1-3AHI. While using Sleepyhead for a while to follow my AHI, I never paid much attention to anything else than the AHI. Just recently, I found this board and am intrigued by the offer of support. When I look at my events, it seems the machine responds to FL but not to either OA or H. I post two pngs: one from last night with AHI 2.37 and one from a couple of days ago with 1.1. Today I don't feel so hot, a little headache and concentration issues. Last Friday I was about ready for a marathon. I appreciate that many on this board would be happy with either number. My doc tells me that anything under 5 is great (I think that's BS!). And I also appreciate that there might be many extrinsic factors, like stress etc, BUT if I could set my treatment into a more effective regime I would be thrilled silly - and make the world a better place by being more focused.<br/>
Any thoughts?

welcome to the forum. unfortunately, i can't see your charts. not sure if this is something wrong at my end or yours. let's see if others can see and advise.

and yes, your machine certainly should respond to both oa and h. so i'm assuming your therapy will need some tweaking.
people say i'm self absorbed.
but that's enough about them.

They're posting with the wrong kind of markup language (that stuff between < and > ) that's not valid for this kind of forum.

As to respironics responding to OA and H... they will, but they're really slow to do so:

okay. maybe they'll come back and do it proper for this forum.

sorry about the non-attachment. I tried the Sleepyhead save screen and missed that somehow those png's got garbled. I now did a 'PrtSc' and saved a jpg. That seems to work. So I am looking forward to ideas on what to adjust.
thanks a ton!

I'd suggest starting by bumping the min pressure up to 7. and seeing how that does over a few days time.

Thank you, Palerider, I will try that and report back in a few days.
I thought given that it stays at 6 most of the time that would be enough, but you make the good point - or at least your comment implies - that this is exactly my problem, the machine is 'stuck' at 6!

Your brand of machine is very slow to respond to changing needs.

Thus, it's very important to make sure that the minimum pressure is high enough to take care of the little things that happen... a little more pressure can hold open you airway better, and hopefully smooth out your breathing.

The more of those pressure spikes you have, the better.

Thank you, Palerider! Do you think the Dreamstation would have a similar response to the System One? Also there is a claim that the video is made by ResMed?
I am one night in on minimal pressure 7. Not the immediate remedy I was wishing for, but I'll keep trying.
My other thought was Central vs Obstructive. I would expect the central apneas to be less responsive?

Yes, the algorithms are fundamentally unchanged between the PRS1 and the Dreamstation, as well as the Resmed S9 and Airsense 10 lines.

Yes, the video was done by Resmed, but it's accurate, you can see it in your graphs, where you're having hypopnes, and obstructives, and the machine does nothing. Resmeds respond to single events, Respironics do not.

What that means is that people end up needing a higher minimum pressure on the Respironics, to prevent those single events.

Thank you Palerider! I wish my sleep Dr would be so knowledgeable!
I had my second night on a 7 base pressure, not great yet, but I remain hopeful

how do you feel when you wake up? are you feeling better with the minimum raised?

I think you're going to need more pressure, If 7 is bothering you, then wait a few more days, 7 isn't bothering you, then bump it up to 8 and let's see if we can get rid of some of those hypopneas.

I am sleeping fine with the pressure raised, but not feeling any better in the morning. In fact today I do have a headache which is a bit unusual, but does happen.
