Full face mask pops up into mouth while sleeping

Post by andycole » Thu Nov 19, 2015 11:55 am

I have been on ASV for approximately 4 months and almost ready to return machine. I have complex apnea and use a full face mask. So far have tried ResMed F10, Phillips Amara View, and now ResMed Quattro Pro. The problem is that, while sleeping, somehow my mouth opens enough to cause the lower part of the mask to pop up into my mouth. No one is sure why this is happening and are recommending I use my albuterol inhaler 4x daily and NOT to use Flonase before bed (use in AM instead). The F10 caused an abrasion on bridge of nose from rubbing up and down. The Amara would leak like crazy when the pressure increased. The Quattro solved the rubbing issue but is still popping up. My machine is a ResMed Aircurve 10 and we have now changed pressures to 5min and 10max as thinking is I am fighting the machine. Have set ramp at 45min, manual startup, and automatic humidity. Having problems with sore throat and chapped lips so may change humidity to manual and increase it. Any help would be appreciated.

Sounds as though you need to try a chin strap to prevent you from opening your mouth. A Hans Rudolf FFM has a "shelf" that the chin rests on, that would also prevent the mask from getting into your mouth. ~~~ https://www.cpap.com/productpage/hans-r ... -mask.html

You could also try a cervical collar like this one :

It work well for me .
If you're not part of the solution you're just scumming up the bottom of the beaker!

Get the Clinicians manual here : http://apneaboard.com/adjust-cpap-press ... tup-manual

I agree with the chinstrap or cervical collar

To keep my mask in place, I place two pieces of tape from the lower part of teh mask down under my chin. I also use a piece of moleskin on my nose to keep the mask from irritating the skin.

I use a resmed quattro small. It has done the jumping in the mouth thing 2 times. I think because its a small and the bottom part rides higher on my chin. I tried the resmed quattro med for several years but had lots of leaks, but it never jumped in my mouth...lol. When I went to the small I started sleeping better...better fit. I guess Im so used to it now, it stays in place...good luck, may want to try a larger mask.

This is a big reason why I do not wear a full face mask.
There are a few (very few) full face masks that cup under the chin.
Perhaps one would work for you.

Fisher Paykel has a couple others; and Hans Rudolph does as well.

This has happened to me twice when I forgot to wear my cervical collar - http://www.amazon.com/Carex-Health-Bran ... GKPR9AJ7HQ

The collar solves this problem, and additionally, it allows me to run at a lower pressure.

got a chin strap but therapist and I can't figure out how the strap would work in conjunction with the mask straps

what does a cervical collar have to do with complex apnea? I did take a look at full-face masks but have to talk with the respiratory therapist first.

They don't. Put one item on, then the other, over top of the first. The order in which you'll do that will be determined by which way feels best for you. I always put the chin strap on first when I was attempting to wear a Swift FX. (didn't work out) I am too claustrophobic to ever be able to wear a cervical collar but, it might be just what you need to keep your mouth closed and not get in the way of your mask and it's straps. Good luck.

we looked at the chin strap and the mask but there is some kind of buckle on the chin strap which interfers with the mask upper arms.

A soft cervical collar may do two things: 1, keep your head from tilting forward which helps to keep your airway open and 2, keep your chin from dropping out of the mask by limiting the distance your chin can fall. If you get one, make sure it is a soft one and, in my experience, the smaller the width from top to bottom is better. I used a 2.5 inch one. Also look for one that is close to your neck size. A one size fits everyone will probably have too much overlap in back to be comfortable while sleeping.

An alternative to a chin strap is a length of stick to itself Velcro about 1/2 in wide. After the mask is on, thread each end of the Velcro from under your chin up under the lower straps of your mask between your cheeks and the mask straps (soft side against the skin) then back down over the straps to close on itself. This Velcro strap does not have to be too tight. Just snug will do. It should help prevent your chin from dropping out of the mask when you relax while asleep.

You need to find one that doesn't do that, then. This whole endeavor is to overcome obstacles, not to be stopped in your tracks by a buckle, etc. There's several types of chin straps available. Even a garden variety Ace Bandage can serve in a pinch.
