Hey folks,

I'm doing better! Still have bouts of daytime sleepiness and cognitive decline -- as if my brain is on a low battery. Insomnia is much improved. In response to a previous question, I am not on any medications.

Raised the minimum pressure to see if that might reduce hypopneas. From 5.0-10.0, I went 6.0-9.0, and now 6.4-8.4. Started lowering the maximum pressure because I wondered if I might be one of those folks experiencing micro-arousals due to pressure adjustments. Also read threads about flow limitations driving up pressure, and I'm not sure that I'm benefiting from increased pressure: there was one night with a median pressure of 8.5 with 8 CA's, 0 OA's, and 0 H's...

Please see recent chart below, and let me know if you have feedback or recommendations. Ignore that last cluster of events after 7:15 AM because I was awake after returning to bed. Thank you!

That is a considerable improvement, especially with the events occuring so close to when you go to bed and when you awake. Take those away, and you've got a pretty clean night except for the breaks in therapy or mask removal. You seem to be on the right track with what you're doing. I'm curious if you have any kind of respiratory issue aside from apnea that might affect your flow volume. Your tidal volume and minute vent rate is quite low. It doesn't mean that much other than it seems to be a fairly small flow volume.

Not that I am aware. I wanted to ask the doctor this week but didn't manage to ask all of the questions on my list. I suppose I could have been more assertive to squeeze them in. Generally, the doctor suggested that the airways are fine. With a low AHI in my initial sleep study, I'm on the borderline of being diagnosed with sleep apnea. So, that leaves insomnia as a greater contributor to my state.

I'm going to continue at xPAP and look for additional improvement, especially with this recent string of lower AHI. If I'm still experiencing daytime sleepiness in two months, then I'll go back with a different tack.

Good thing the social calendar is a bit sparse this Friday night: I found ranges of 'normal' tidal volume and minute ventilation. It seems I'm at the low end of 'normal' in terms of the median reported by Sleepy Head. Following along on the charts, there's variability.

"Normal tidal volumes are about 5-7 mL/kg ideal body weight."

"Minute ventilation depends on body size and metabolic rate. Normal VE values range from about 5 to 10 L/min in resting adults."
