So, "IMHO" what you have there can perhaps best described as "mess". That said:
  • See comments about stroke. Post-stroke mess is not uncommon (BTW, two-thirds of stroke patients get SDB).
  • While ResScan does not identify much flow limitation or snoring, most of the inspiratory waveforms look quite ratty and might/should benefit from some pressure support. This would be best delivered with an IPAP attack (BiPAP or ASV).
  • That generally appears to be the extent of the obstructive component. The apneas themselves seem to be generally of a central nature (although this is already on some CPAP), thus requiring a back-up rate of some sort (BiPAP S/T or ASV).
  • The central apneas do not appear to be of a periodic nature-- rather they are irregular (and it is important that we call them exactly and technically that-- irregular respirations. ASV was designed to break the periodicity of chain central apneas, not charge through and hammer each one into submission. That said2, that may end up being the approach here).
  • The reason we need to consider hammering them is because they caused all those oxygen desaturations.
  • This not Complex Sleep Apnea. I believe the original HST was correct in the report (CSA, or mostly CSA) but incorrect in the interpretation (OSA).
  • This continues to support our current plan, namely, grabbing Esky by the throat and screaming in his face from a distance of about 2 inches:

    "Just WTF are you trying to DO here!?"

I love your approach. There is no doubt in my mind that you have worked as a sleep therapist or perhaps have a degree (MD) of sleep disorders. Anyhow, thank you so much for your efforts on my behalf. When can I expect a bill?

My aunt and I just got back from seeing my primary doctor. She is going to call ESKY herself and tell him to revisit the test and give the correct interpretation. Let's hope we get the right resolve.

I have a question that doesn't relate to apnea at all. How do you guys get parts of a previous reply into the reply you're composing at the time. I answer questions and "hope" the person asking, knows the reply is for them. Help

You can do the quotes 2 ways...
just hit the quote button on the comments from the person you want to reply to and you will get a new reply box pop up with the the quote already included in its entirety.
I don't do that for large comments but I might do it for a small comment. I don't see the need to quote a big long comment myself but some people prefer it that way.

Personally I like to just quote the main part or a question I am replying to...like this

I just scrolled down below my reply to box here that I am current typing in and have open and highlighted just that line and then clicked on the quote button and this is what I get.

I will do this a lot when someone asks multiple questions in a long post and I want to make sure my answers are clear..
so then I will quote one question...give an answer then quote another question and give it and answer and so on.

Go ahead and play with the quote button to see what you like the best. If you mess up you can delete your own post as long as someone hasn't responded after it and if someone has you can still go in and edit it.
