need help with a nursing school project

Hi all,

I am a PhD nursing student from Illinois State University Mennonite College of Nursing. I am working on a class project collecting data to construct an instrument regarding functional status as it relates to tiredness. I am in need of 50 individuals to fill out a questionnaire. You do not need to disclose your name. The only demographic information I need is age range as I am looking for 25 individuals 64 years and younger and 25 individuals who are 65 years and older. It would also be helpful to know your gender and ethnicity. I am attaching the 17 item questionnaire. It should take less than 5 minutes to complete. This data will NOT be used in any formal research or presentation. It is only a class exercise to further my education. Thanks so much for participating. I am expecting to have some trouble collecting information from the 65 year and older crowd HOPEFULLY you can prove me wrong. If you prefer, I can send you a word document to complete, please send me your email. I will NOT share your email information with anyone and will delete once I have your completed information or within 10 days (when the project is due). Are you relating this to OSA? If so, before or after therapy. Is this regarding any cause for 'tiredness'? Please be more specific in your request for info.

Have you cleared this questionnaire with your Nursing Instructors? How about your IRB? This type of research is tightly regulated at the University level. Trust me; I do know what I am talking about. Although you may well be exactly who and what you say you are, surely your Nursing Dept Instructors would be among the first to advise you against asking for email addresses in this situation. And yes, I do know you said you would discard them. I do not mean to be harsh and I apologize if I have offended you. Just please (if this is what you say it is) make sure you have the approval of your Nursing Dept to move forward.

Connee is finishing her PhD in Nursing....I've PM'd with her a bit today. Seems she is on the up and up.... I did offer some suggestions on how she could have done this questionnaire a bit more easily without needing email involved. Using Google Docs Forms, she could have constructed a good survey and posted the link here. The results would then be aggregated automatically into a spreadsheet for her.

I suppose she may have posted here before.....as she is at the end of her doctoral program, and those take years to do.
