PS of 4 is what the labs commonly start out with and most people come out of a bilevel titration with PS of 4 or 5.
On a rare occasion a little more for some reason or other.

My suggestion is start with PS of 4
use auto mode...
Minimum EPAP 17
PS of 4
Maximum IPAP 25

See how much a little more IPAP helps with those FLs before adding more EPAP....might help out just enough.

Ok - I was going to restrict the max pressure just to start with - I have this irrational fear that the machine will shoot straight up to 25cms and hang out there. I was trying to feel as though I had some head room for a day or two.

Oh...it also will have some other comfort settings.....use the factory defaults first and see how you do with them.
Most people won't need to change those but if you have trouble getting in sync with the machine rate...then we can change them.
When I was using the VAuto S9 I did just fine with those default settings.

It was a used machine that I bought from a forum member for a friend who needed bilevel because her pressure needs were like yours and she had some COPD and was struggling.

She wanted me to use it first to make sure it was working like it should and set it up for her.
So I did.
When I sent it to her I told her that if she didn't like it that I would gladly buy it back....she got the machine and used it one night and the next morning the only message I got was "over my dead body"

So - trying hard not to be overexcited, but thanks to LSAT, I have a tracking number that says I will be the proud owner of a Resmed Vauto tommorrow by 8pm!!

I am planning to start at comparable settings to my current APAP which is running at min 20, max 20, EPR 3.
Can someone check my math please?

1) Should I be putting Epap 17, Ipap 20?

2) All the titration protocols say PS should be at 4. Is 3 OK?

3) Should I be doing any other kind of settings?

4) What is the next change to make? I'm trying to limit FL.

What I would suggest is:

Put maxipap on 25, and *trust the machine*.

I am not one of those - I am one of the ones who let the max be 20, and found the machine bumping against the top of the graphs.

Just for a few days I wanted daylight above the max pressure line.....

Try ps 4. You will like it. I tried 3 and it sucked.
you can always back it down if you don't like it. It doesn't seem like it would make a huge difference in comfort but it does.
