New CPAP User about 35 days; What went wrong last night

Post by sleepbetter » Thu Jul 26, 2018 12:19 pm

I've attached a screenshot from last night. Any help would be appreciated. Not sure if I should consult DR.
Thanks

Your minimum and Maximum pressures a are too low..Raise the minimum to 7 and maximum to 15. Try that for a couple nights and then come back

What was your typical AHI the other 34 nights?

Future charts will be easier for us to read if you use the format in Pugsy's SH tutorial - viewtopic/t88983/Pugsys-PointersSleepyH ... Hints.html

I believe before last night and a really high nigh about 5 days ago I had a running average of around 4 AHI.

Thanks

Did you happen to maybe sleep more on your back last night than you might have done the other nights?

For some reason last night you needed more pressure and the machine couldn't go higher because of the tight range.
Something changed last night and the first suspect in my mind....supine sleeping.

If you look at the pressure line on either graph, you see it is running up to 8.0 and staying there most of the night. That is why they are telling you to raise the max pressure - the machine needs to go higher to keep the airway open.

About the two very high AHI nights, did you sleep in a different position? Have nasal congestion? Eat or drink very late? Get plastered  ?

Since you did so well for most of your nights, I would raise the max pressure in steps. 10.0 for tonight, and let's look at the chart tomorrow.

I don't particularly like making more than one change at a time unless unavoidable and urgent.
Just increasing the maximum might be enough to deal with things.
See what happens once the machine's hands aren't tied to 8 cm max...then maybe increase the minimum if it appears to be needed.

Thanks; I will try to 10.00 pressure tonight.

I did sleep on my back last night a lot. (I believe). Had no alcohol last night and a very reasonable light supper.

That sounds good.

Unless you experience any of the following, I see no reason not to turn the max to 20 (which is as high as it goes):
1) An increase in "Clear Airway" events that appears to be associated with higher pressures.
2) Leaks that are annoying or that are beyond the capacity of the machine to compensate. Remember that the machine will compensate for minor leaks, so if you can live with them, they're not an issue.
3) Pain or discomfort associated with the higher pressures.

Here are last nights results. Thanks to everyone for the help. Thank you pugsy for the information on posting and navigation of the site. Not sure why pressure stopped at 7.0.

I think maybe you have EZstart enabled. Let's go into the clinical setup menu and turn it off and just use regular start.
EZStart keeps the pressures low so you can hopefully get used to it easier but it's not doing you any favors because you need that higher pressure now.

Go here and request the clinical manual for your machine via email.
https://www.apneaboard.com/adjust-cpap- ... tup-manual
in are the instructions for how to get into the clinical setup menu area to change or turn off EZStart if you don't already know how.
Plus explanation of what EZStart does. Along with some other really useful information about your machine that isn't in the user manual.

Eventually it will let you work up to a higher max but it takes a while and while it is doing the slow increase your apnea isn't being treated. EZStart is a feature that is simply stupid IMHO especially when the max setting is really fairly low (as yours is) anyway.

It is kind of strange that it stopped at 7. I guess that there's no point of turning up the max (at this time), as it seems to stop at 7 anyhow. The only thing that you might want to do is chedk that you're settings didn't get changed before you go to bed and after you wake up.

Remember that your DME can (at least theoretically) change the settings remotely.

Oh...no need to include the respiration rate graph. It's not particularly helpful.
Also no need for the snore graph since snores are easily spotted on the Events graph.
Omit those 2 graphs and resize your other graphs (tomorrow's report once EZStart is disabled..no need to redo this one) so the other are a bit larger and easy to evaluate.

Using EZStart will cause it to happen. Somebody turned it on and it causes more problems than it fixes.
