Settling is the terminology used with the Resmed Auto machines.
Well, not exactly. The ResMed has a ramp feature in cpap mode, which functions like other manufacturer's ramp feature... it gradually increases up from a given starting pressure up to the the single given therapy pressure over a given period of time, to allow the patient to get off to sleep before full treatment pressure is applied. Settling is used in apap mode, and it starts with the minimum given pressure and stays there for the given settling time, ignoring apnea or flow limit events until that given time has expired, then begins to respond to flow limitations or apneas with pressure increases up to the maximum given pressure. It does not gradually increase pressure at all during the settling time, unlike the ramp mode, which does.

You have a good machine, and I trust it will serve you well.

Hope this helps!
Yes, I understand that... however since the machine is being used in AUTO in this case I did not want to confuse the issue.

Okay... how did the night go?

Yes -- I'm anxious to hear too. How did it go?

I've been using an AutoSet II for about two months, and everything has been going fine.

However, something popped up recently -- I got a card reader, card and ResScan, and I downloaded my first set of data (it is one step more complicated than that -- I have a Mac, but I run XP under something called "Boot Camp" -- the good part is that I had absolutely no issues with the software or reading the card). The summary graphs are exactly in line with what I've been writing down every morning.

But -- I get a message that says "detailed data not available", and I see the summary data. I've been looking through the clinical manuals for the AutoSet II and with ResScan, but I don't see anything about enabling detailed data.

The bottom line is that at this point, I'm not sure if detailed data is even available for this machine.

Anyone able to educate me?

thanks,
Guy

Guy, when you d/l your data you may need to click the box to get it to d/l the detailed data. It should do up to 5 sessions.

Well last night was well, okay. I expected to feel totally different this am but I feel the same. AHI was under 5 but I really think I need to bump up the top pressure a bit. I did wake up at some point with a sore arm because I had been laying in the same position too long, so I think that is a good sign that I slept more deeply than normal. I do think I am going to increase the top # to 12, because I want more benefit than I got last night. The one good thing is I did not have the big clusters of events that I normally see. There weren't near as many together. So things are moving in the right direction.

Thanks for following my saga guys!

That reported AHI is very good and encouraging.

Did you check on the AI & HI?

Keep in mind with the Resmed that you pretty much need to cut the HI in half to get a realistic number since the machine scores hypopneas very aggressively.

Where did the pressure spend most of the night? Did it bounce up against the max range number and stay here? If so then yes you should adjust that number upwards but don't be too quick to make changes... give yourself time to adjust and the machine to do its thing.

Thanks, 2girlsmom, I'll check into that tonight.

BTW, although the mechanics of therapy have been going fine for me, I don't always feel a dramatic effect either. There are times I still feel groggy on waking and mid-afternoon, although I think that things have been better overall. I know I'm getting REM every now and then, and while I used to frequently wake up in the middle of the night and not be able to get back to sleep, it seems like that hasn't happened since I started therapy.

I recall many people on this forum saying they didn't feel like they were comfortable until about the third month. I'm looking forward to that.

The last time I visited my PCP, which was about week 3, I had a dramatically reduced blood pressure. Lowest number I've seen in years. So I bought a B.P. machine so that I can monitor it. Overall, I think the numbers are fairly good. I hope to make a case to my PCP that I can come off my B.P. medication (while monitoring, of course) or at least reduce the dosage.

Median pressure was 10.8, 95%ile was 11 and max pressure was 11. I went ahead and put max at 12.

AHI was 3.7 with an AI of 0.4 and HI of 3.3. So it is good, but I went to bed late and was woken up at 6:30 by my 5yo. I still got some clustering of events even @ 11. I did wake up a few times as well. It just wasn't the magical night I had envisioned. I do think we're headed in the right direction so that is encouraging.

Thanks for sharing, 2GirlsMom!! Maybe not a miracle occurred but things are looking and sounding good. Just hang in there. Patience is a virtue.

I'll post in the morning about my night tonight. My husband is off so I may even be able to sleep in if I need to. I am looking forward to seeing how increasing the max to 12 will go for me.

Hey, DSM may well have hit the nail on the head! I know my first two titrations I was so excited about finally getting som RELIEF and some SLEEP I couldn't sleep for beans. And worse than you - I only slept 48 minutes out of 6 hours bedtime so they brought me back in for another titration and I only slept 98 minutes out of 6 hours bedtime! And its NOT like I hadn't had some experience sleeping w/a mask and pressure and wires and all.

I think adrenaline did keep me from sleeping well. And, I drank plenty of caffeine. Today I had only one soda and it was very early so I am confident I will konk out when I head to bed shortly.

As i understand it, the S8 Autoset only stores 4 sessions of detailed data, so you have to download at least every 4 days. Once you've downloaded it and saved it, of course you have it forever.

Go ahead and put the Maximum up at 20 cm, no use letting that getting in the way to skew your results. If pressure during the night bumps into that 12 cm pressure it will throw off the 95% data and the rest of the data won't be that accurate either.

Since you are already over 10 cm pressure, it is only going up to 11 and higher due to Flow Limitations and Snore. FL's will show up under HI reason they are always higher than compared to other machines.

What you should go by is Leak, and AI. Get AI down to about 1 by increasing the Minimum pressure up and you'll have it, once you get AI=1 then start fine tuning in .2 to .5 pressure increases until HI drops below 5 and you will have it. Leak must me within limits for any of the data is to be considered accurate.
someday science will catch up to what I'm saying...
