The name of the game is NOT to get the AHI as low as possible. The name of the game is to get the best sleep.
...
But for many people, there's a lot more to optimizing this crazy therapy than merely chasing a 0.0 AHI. Comfort is an important issue: If you are not comfortable, it is difficult to get a good night's sleep.
...
In my opinion: 'Tis better to get some really high quality sleep with a slightly higher AHI than it is to sleep poorly with a perfect 0.0 AHI night after night
Very true, and that was part of the reason I tried to make it clear that she really didn't need to chase lower numbers and there were other reasons she might not want to. However, this particular member was running 10 - 20 APAP previously and was happier with that setting, but she was recently scolded by her DME and had her pressure lowered by them to 9 - 12 citing her doctor's pressure recommendation as the reason. She has been sleeping at the new lower pressure for several nights but does not feel it is an improvement.

The real issue here isn't the 9-12 pressure vs the 10-13 pressure. She was happier at 10-20 but after getting scolded for it she is looking for support and validation that she wasn't wrong in doing what she did.

Thank you both! I thought it was me being dense.....thank you for correcting your post, djhall. I am the competitive sort and have to force myself not to chase the numbers. The thing is I felt very well at the beginning of therapy and now feel worse especially since the change in pressures.

Pugsy has told me in the past that I need to give the settings time which is why I decided to give the drs setting a week. If I don't feel better at the end of the week, I will try it at 10-13. I think that will be much better.

Robysue, do you sleep well now? You sure had a rough time of it!

Right now I'm cautiously optimistic that the Second War on Insomnia is (finally) close to being won or perhaps it now has been won. The Delayed Sleep Phase problem is not a serious problem as long as I don't let the sleep schedule get too far off what I need it to be.

Back on March 3 things weren't going so well. (See this long thread called Fractured sleep, Ambien, dial wingin' and other things.) But shortly after posting that, things started turning around. I posted an update, A (very) positive update to my March 3 thread, on March 29. Since then, things have continued to be pretty good. I still sleep some crazy hours at times, but the sleep now feels like good sleep in the sense that I wake up feeling rested and sometimes even almost (dare I say it?) refreshed. I am still having some problems concentrating during the day, but I think that's being brought on by some plain old stress issues that have nothing to do with sleep, but do have to do with job and extended family problems.

Well I'm very glad to hear that. Hopefully, things will keep improving! I will check out your thread .......think I skimmed it before!
