using a cpap diagnostically?

Post by ironhands » Wed Jan 29, 2014 9:47 am

So yeah... I scored a 0.0 on my first PSG, and a 16 on my second. I'm currently using a trial machine, set to a pressure of 7, and I'm getting an AHI around 3 (last night was <1!)

I'm wondering if I should tinker with lowering my pressure to the lowest, just to see if it's doing anything at all. Is there any point, if I'm still seeing events with the current pressure level? I'm still concerned there are other sleep issues going on. I'm having a titration soon, do they still do the EEG portion of the test then to check for neural arousals? If not, is there really a point in doing a titration when I will have a month of APAP logs?

I would not lower the pressure, but raise it to 8 or 9 and see if your AHI goes down and you feel better. It's not just the EEG that's different in a lab btw, but other electrodes record, or at least raise issues, with potential central apneas, other sleep disorders that aren't apnea, etc. etc. So while you can use an Apap to titrate, it's best to first have a solid (recent) diagnosis of obstructive apnea before just going ahead.

You need to figure out the event category breakdown for that AHI.
Remember...if mainly centrals we do nothing....because pressure isn't used to try to reduce centrals (clear airway).

For all you know that AHI of 3 could be 2.9 centrals per hour average because of a cluster of them when you were maybe awake.
Use the daily detailed reports to see exactly what you are trying to fix.
Don't go changing pressures based on AHI all by itself....

Thanks Pugsy. I've only seen 1-2 centrals showing up over the past week in total, and those might have just been when I woke up in the middle of the night (still getting used to the machine) as you suggest.

Logging everything into sleepyhead, I'll take a closer look at it tonight when I get home, but so far my centrals have been very, very low. I didn't check it for last night yet (it was under 0.5 AHI though)
