Have you checked to see which other online suppliers are available? If not click on the link at the bottom of ChunkyFrog's signature box. With the hassles you're having with Crapria I would quit messing with them and go to another supplier. Since you're dealing with Crapria mostly online and phone now it doesn't matter if your supplier is 2 miles or 2000 miles from your home.
Your doctor's office may be irritated at having to send documents again but I'm sure they'll understand.

That is why I do NOT let anyone send documents to another someone.
"I" get the needed documents myself, make sure those are the documents I really need, then I transmit them to the next party.
Cutting out the middle man always takes out all this BS about X did not send to Y what Y asked for.
I deal with Apria (for someone else) using that procedure and I have NEVER had any problem at all with them.
