Anyone Tried PureSleep?

Post by locust78 » Thu Oct 18, 2012 6:11 am

It looks like an interesting product. It would be nice if it actually worked for cases of mild to medium OSA, so that it could be used for travel. I'd love to pack up a little item like that, instead of hauling my whole system around when I travel. I have tried a different brand of mouthpiece in the past with mixed results. Has anyone tried this one?

It has been discussed on the forum in the past. Haven't seen any discussion lately though. From what I read in the past...not good.
You could search the forum for discussions about it...2009 it was discussed. I remember that much.

My hubby wears this every night - for snoring ONLY.

And it works like a charm - he says it's very comfortable to wear all night long, but was shocked that for quite a few of the first 30 days, he woke up with the device on the floor LOL. I told him it was common with XPAP users, so no reason the same wouldn't apply to his mouthpiece.

Btw - DH doesn't have OSA - his breathing/snoring is very consistent and regular without any hesitation or periods of no-breathing.

I guess for a few people this might work for very mild OSA to push the jaw forward a little, but IMHO it might just stop the snoring, falsely making the user believe that their OSA is also stopped.

Cheers,
xena

Not to be confused with Pur-Sleep

PureSleep(R) is a mandibular repositioning device (MRD). All MRDs work by advancing the mandible (jaw), thereby holding the associated soft tissue forward of its normal position and away from the soft palate. This opens the upper airway allowing more unobstructed breathing, which is what causes both snoring and OSA. There is a large body or researach showing that MRDs are effective at treating snoring and obstructive sleep apnea (OSA). However PureSleep has only been cleared by the FDA to treat snoring.

I spoke with a dentist who is VERY lukewarm about it.

I have tried a (pricey) jaw-advancing mouthpiece before. It seemed to give me ok results, at least for the short term. The downside was that my teeth ached a bit in the morning, my jaw felt out of position, and it was somewhat bulky which meant that my lips couldn't meet properly and create a nice seal. This resulted in a my mouth feeling drier than the Sahara in the morning.

There probably is a place for devices like the PureSleep device with some people who have OSA. It might be enough to maintain an open airway with minimal collapses and it might not. Probably a lot like the Provent nasal plugs.
If I wanted to use something like either of those products I would want a sleep study using them so I knew exactly what improvement (if any) I was going to be able to achieve. My OSA is too severe and my oxygen levels dropped way to low pre cpap to go by guessing. I would want cold hard facts that were indisputable and going by someone else's studies or results would not be acceptable. I would want my own results with my own sleep study.
Most of what I have seen for these type of oral appliances and even the Provent is maybe 50% improvement for some of the people.
In my case that would still leave me with AHI in the 20s and likely desats well below what I would be comfortable letting happen.

Now someone whose pre cpap AHI was maybe 10 and oxygen levels never really dropped very much and stayed maybe in the 90s..maybe a different story but I still think I would want a sleep study using whatever device...I just wouldn't want to play guessing games with my health. The absence of snores doesn't mean no OSA. Not every one who snores has OSA and not everyone who does have OSA will snore.

Pugsy, thanks for bringing me back to earth. I was entertaining the idea of trying one of these again; popping it in my mouth & if I 'felt ok', I'd go with it. My OSA was of the sort that I'd regularly wake up gasping. If the mouthpiece stopped that, I might have concluded that it's working, even though less dramatic but just as dangerous events would still be taking place...
