
0 centrals in lab, many at home
0 centrals in lab, many at home
On my fourth night of home CPAP use, I'm getting a lot of central apneas (about 4.5 an hour). In the titration I didn't get any. Any good interpretations?

How is your overall sleep in general? Are you waking often? Having trouble going back to sleep if you wake up?
Spend much time laying in bed awake or semi awake with mask and machine on?

I've been waking up 4-5 times each night, though I have no trouble going back to sleep. Initially, it takes me 15 to 45 minutes to fall asleep. I did feel better than I had in weeks, on the day after the titration, so I'm puzzled as to why my experience with cpap is so different at home.

You are waking up 4 or 5 times a night that you remember and I bet there are more that you don't remember. That alone indicates fragmented sleep and fragmented sleep messes with normal restorative sleep cycles and will sure mess with how you feel the next day.
Do you know what is causing the wake ups? Mask, pressure or is this something you did prior to starting on the machine and maybe pointing to some sleep onset insomnia (taking 45 minutes to getting to sleep) and/or sleep maintenance insomnia (waking often and not staying asleep)?
In the sleep lab the tech was able to watch things really closely and fine tune things as the night went on.
We can't do that at home. Some people get lucky but most of us have a bit of adjustment at home. It just isn't normal to have to sleep with the mask attached to our face.
Try to isolate why the wake ups...see if you can correlate some of those central clusters with the wake ups.
Work on the cause of the wake ups if you can. Look at the other things that can mess with how we feel and how we sleep besides just the sleep apnea part of things. Often there is more going on than just sleep apnea.
Tell your concerns to your doctor and see if he is concerned. He will likely say "give it time" and there is a lot of truth to that. Until you can sleep without remembering many awake ups we can't really evaluate these centrals because I suspect a good many of them would not be scored in a sleep lab because they don't meet the criteria.
