3 days after switching to APAP, feel bad or worse

Post by forgetcolor » Tue Jul 17, 2012 12:35 pm

Switched to APAP 3 days ago. Study titrated me at 5cm. Didn't work. Switched to 6cm w/ doc. Same. Tried 7cm on my own. same. And now just switched to APAP, 6-10cm, EPR of 2., EPR of 2.

I only have data for two of the last 3 nights (forgot the card), but the two nights tell a similar story. I know 2 is earlier to draw a conclusion, but thought I'd get some early feedback.

Last night, as I often do, I woke up about 5-6 hours into the night. What's interesting is that at this point I felt okay. I felt too tired but I thought oh, maybe this is finally working because I don't feel dead. And then I went back to sleep. 1.5 to 2 hours later I wake up feeling like a zombie. Super tired, that dead behind the eyes feeling.

Looking at the data, I had 38 CAs last night, but 33 of them were in those last 1.5 hours. I know CAs can happen during the last bit (and during sleep onset), but is this normal? I also see that the machine cranked the pressure in response. I thought pressure was bad for CAs? The rest of the night, as I felt, was really good for me.

Clearly the APAP is doing its thing, going up to 10 a few times each night. But I'm concerned about the CAs and wondering what I should try next. I think I should probably just give this setting more time but am interested in your perspectives.

thx!

oh, and before someone asks, yes my sleep time is way crazy. i have a severe circadian rhythm mismatch w/ the world, and in summer I drift into the 4-5am sleep time range.

If your machine is responding with an increase in pressure it is in response to either a flow limitation or snore or other obstructive precursor and it is just a coincidence that a central event was recorded at about the same time. It is NOT responding to the central but most likely responding to something it sensed that you don't see on the graphs. They just won't do it.

If you happened to be awake or partially awake during those clusters of centrals then likely the machine was fooled by your awake breathing which is much more erratic than we realize and easily fools the machine.
Given that the bulk of the night was really pretty acceptable and the last portion shows things all going to hell then I suspect that those clusters of centrals are most likely not "real" centrals but very likely either awake or semi awake events being falsely flagged. I would suspect that if you were in a sleep lab that those clusters would be totally tossed out. Of course there is no way to tell for sure but if you were having real issues with "real" centrals then the first part of the night would very likely have a whole lot more of them showing up.

Why do you feel like crap? Unknown unless it was just really poor quality sleep that you got after the wake up.
