G'day folks,
Just wondering if anyone could relate their experiences of not feeling better or worse on CPAP (even though your apneas reduced) but having better luck with VPAP/BiPAP?

Yes.
The second night at the sleep lab, where I was put on CPAP, it ruined my sleep and I was a wreck. All of the day following the CPAP titration at the lab, I was miserable: I did not feel well-rested,– I felt very tired; all day, I felt drained and worn out; my nose ran continuously all day and I couldn't stop it; the front portion of my head felt like someone had drilled holes in it; my reasoning processes were slowed down for the entire day; all day, my eyes felt irritated and too tired to read my book; and I felt dull and cranky and very grouchy.
The report from the lab said that my CPAP treatment during the test "did not improve my sleep architecture and efficiency" - an understatement for sure, and that my diagnosis included central apneic episodes.
On my third night's visit, I was titrated on ASV treatment, and thereafter was approved by Medicare for VPAP, on which I believe I am doing very well.

Yes, with initially over 60 days of S9 Elite straight CPAP at 10CM h2o, I consistantly had AHI between 8-10 and averaged CAs around 6 - 7.
Now, with about 30 days on VPAP Adapt SV, AHI is under 2 and AIs are in the .5 to 1.5 range and MANY nights zero events.

What if you're on an auto, low numbers, great on paper, but some centrals and feeling bad, 7 years in?

We look at how many is "some centrals" and if not in great enough numbers to take the entire blame for feeling like crap we get to look at other factors affecting sleep quality, quantity, and other potential culprits unrelated to sleep apnea..which the machine can't fix.portiemom wrote:What if you're on an auto, low numbers, great on paper, but some centrals and feeling bad, 7 years in?

I am concerned about CPAP machines. First, I was told later that CPAP machines can cause central apnea which is scary as heck!! Also, depending on the DME, which I have little faith in, if you have 8 AHI's in an hour throughout the night, it is acceptable. I find that not acceptable at all. Also, a big concern of mine is, once you have a sleep study, who is supposed to monitor you to make sure that level setting is correct? I finally got told, after spending a lot of money going to a respiratory doctor, etc. that the only way to really know if the machine is doing what it needs to is to have another sleep study. In 9 months, I still do not feel rested at night, I still have memory issues, etc. and I am beginning to think that the majority of those issues are because 1: I do not have apnea 2. I have apnea and the machine that I have is not helping 3. that I have another type of apnea.
Someone I was talking with the other day said the CPAP did not work for him and so they gave him an oxygen concentrator. I don't really know much about that but if I have apnea, I would want something to keep my airway open as well as make sure there is enough oxygen. I do have to admit, I do NOT trust medical personnel. I watched them swear they were giving my husband the potassium drip that never went down and when moved to the cardiac floor, the drip had never been started and my husband could have had a heart attack; secondly, my daughter just died and the hospital was negligent and caused her death and now 2 boys are left without any parents.

Usually BiPAP is used when cpap pressure gets too high to be comfortable. You can usually achieve the same results with a lower expiratory pressure on a BiPAP vs a cpap with high pressures. When I worked at a sleep lab our policy was that if we were titrating someone on a cpap and the pressure was 15 cwp or higher we would switch them to a BiPAP. It was more comfortable for the patient.

I just did a Bi-pap study about a month ago and my base pressure is 16 and up to almost 18 at times on Apap. I found I couldn't adjust to the way it felt. I have been using Apap/Cpap for 6 to 7 ish years now so maybe I was just too use to pap function. For me the Bi-pap felt like it would prematurely cut off my inhale, I would get in about a half a breath and the pressure would cut out, it felt like I was only getting in half breaths. They turned off EPR but it still sucked.
I have used Cpap, Apap, and now Bipap and still favor Apap type machines for myself. In Autoset mode my AHI is normally under 2.0 and when used in Cpap mode base setting of 16cmH2O it climbs to as high as AHI 8.0.
I do think though if the BI-Pap would "wait" for me to finish my inhale and follow "my" breathing pattern, the exhale pressure settings would be very nice to have as well as a higher top end.
So this surprises me. Isn't that a setting on the machine? Which bipap were you using? This was in a clinic Sleep Study. The unit was an Remstar if I remember right this was a month ago or so.

I am pretty sure this was the machine. I did wonder if they had something screwed up tbh. I never have had to try to "match" a breathing pattern to get a full intake of air before and it was really wierd. Oh and it was Flex they turned off, sorry EPR is my S9 term. They tried it on for about 2 hours and I called them in and told them what was going on and then they turned it off but it was still cutting off on the inhale. Finally they just set me to Apap 16cmH20 average pressure after that.
