I would also include a list of states where there is no death penalty,
travel agencies from which to purchase one-way tickets,
and shipping instructions for firearms and ammo.
No joke, this is a serious back-up for a lot of old folks.

Also, if you have real estate, you can leave it whomever you please; and not have to sign it over to pay for extended care.
