While I had 6 hours of CPAP sleep, I had another 2 or so of non-cpap sleep. I got up, went to the bathroom and came back to bed. The numbers are fragmented, I wake up way more with the CPAP then I did without, lots of tossing and turning.

What you describe is common.

If that 2 hours of non cpap sleep was last then when you got up your body was just coming off the stress of 2 hours worth of OSA untreated. Guess what it will remember first?

Give it time. Try to figure out what is causing the awakenings and do what you can to limit them.

While my history is really not relevant to you I will offer just a bit as example. I have some pretty bad pain issues in neck, back an pelvis. Old injuries in an old body. Started therapy and despite "good" numbers I didn't feel as good as the numbers seem to suggest. I woke often with tossing and turning pain. Finally worked up to 7 hours of sleep but it was horribly fragmented. I still wasn't getting the full restorative sleep cycles. With time, meds, change in beds I finally have some nights with only 1 or 2 awakenings. I noticed the difference when this change happened. It took 18 months to sort through things to finally get there.

So while the body is healing work on never allowing yourself to go back to sleep without machine if you wake during the night for any reason. Try to isolate cause of wake ups and limit them. To have any chance for measurable improvement this needs to be done.

I have been on CPAP for years and 6 hours w/CPAP and 2 hours w/o would make me feel like crap the next day.

If they did a full night diagnostic sleep study, what did they find in terms of AI, HI, time slept, etc.?

Just an update. I dropped to 11 and my AHI numbers were consistent or better then they have been, so last night I dropped to 10...and frankly slept a bit better. I forgot to put the data card back in last night before I slept...so I will need to pull the report tomorrow. I am starting to believe an Auto is the way I need to go (if I understand the differences correctly).

Side note, the DME keeps calling me "representing" the doctor...really annoying. I was speaking to him about some other things that were going on and wasn't aware he didn't work for the sleep doctor until the third time he called.

If I understnad the detail you are looking at, what it means is that the central was 11 seconds long and the unknown was 10 seconds long.

One thing I think is just as important as you lower your pressure is to look at those duration numbers. If yuou start getting more obstructive apneas and they are getting longer, then you may have to slightly increase the pressure.

You have very desirable numbers and pressure. There is a lot of merit in getting enough sleep because you may never feel better even if you never have an apnea if you don't get enough sleep.

Something else to keep in mind about great numbers, if you aren't sleeping good/deep you will probably have great numbers. The deeper you'r esleeping the more the tissues in your throat relax and collapse.
The numbers aren't the goal. Feeling rested is.
I had a higher AHI than normal last night but I felt better all day. My data looks like a solid hour of uninterupted REM. Now if I can get these leaks under control.

It's an assumption on my part because the pattern consistantly starts right at the 1 hour mark after I fall asleep and cooresponds with lower O2 and busy heart rate. It never lasts an hour though! Most of the apneas I had were early morning tossing, turning and fixing leaks.
