Seems like for the first three to 4 months (averaged 4.5 hours a night) I had a heck of a time with my ASV blasting me awake with pressure pulses. I finally went to my RT for help and we tried taking it off auto BPM and setting a rate lower than what I estimated was my lowest breathing rate. After a few false starts (he first tried 15BPM) and sleepless nights I ended up with a min BPM setting of 8. I've not had the sudden blowing pressure pulses since. So, maybe having a min BPM setting is the trick and I am averaging 6.5 hours a night. But I still see in my charts every night I start off at low BPM and close to a liter per breath but within half a hour I am breathing half a liter and 18 BPM for the rest of the night. I've also found that sleeping supline consistently gives me the lowest pressure excursions and AHI. I get tired of it sometimes and will roll over on one side and immediately even when sleepy and sleeping I can feel the ASV having to work harder to keep me breathing. So, I eventually roll back on my back and then it settles down to almost unnoticeable breaths.

I like my nasal mask because of the good fit I get. The forehead bridge keeps the pressure off the bridge of my nose. I have a big head but they gave me a medium mask. I tried a large mask thinking it would feel less flow constrictive but it leaked like crazy. I can't imagine all the trouble you guys have to go through to get an even larger full face mask to fit without leaking. I wake up every time my mask leaks a bit but I guess one could start sleeping through it if it happened all the time.

I am eager to see ResScan graphs from Resmed ASVs if they get rid of central apneas like these, and make up the lost tidal volumes b/c of the apneas?

Well I think that I have found an ASV conversation with a ResScan graph:

WOW, inspiratory pressure above 20 cm!
I don't see separation of apneas to obstructive or central.

Ozze - How did you start with a Resmed VPAP Adapt SV and get to switch to the S9 5 weeks later?

I don't know which one I might be "handed" next week. If I get the Resmed VPAP Adapt SV and take it home, am I stuck with it or can I too ask them to switch it to the S9 weeks later?

I don't know what to expect.

One reason we try to help new users is to avoid having someone be "stuck". If you are handed the older version you are basically purchasing it (though medicare may require a rent to own arrangement). However, when you sight that paperwork, it becomes your unit and they are less likely to exchange it.

You should be offered the S9 unit. That is the newest on the market and replaced the older version.

However, if you can work out a lower cost, then the older version may still be available for purchase. There are some differences (especially on the reporting) that make the S9 better. But the therapy is essentially the same. I've used the older model for a couple years now. It works well.

So, if price is an issue you might be able to see if you can purchase the older version for less. Medicare might preclude that. However, I do not *know* what the requirements are on that side. As I mentioned Medicare might require a rent to own program for one of the ASV units. Essentially this allows you and Medicare to be certain that the unit will address your needs.

I hope that helps clarify this.

When they brought me the Respironics Legacy model ASV, I simply refused it and said that I wanted the System 1. A week later, I got my System 1 after my NP "translated" the pressure Rx for the new machine. Refusing is an option!
