Fred, the thought occurs since you wife isn't particularly enthused, happy or comfortable w/a chin strap, has either of you considered a full face mask? I'm thinking of the Quattro FX. I HATED the original Quattro but am VERY HAPPY w/the Quattro FX. And take my word for it, I am one who has from day one been a firm believer in LESS IS BETTER. The Quattro FX is very small for a full face mask and even lighter weight than some nasal cushion masks. There is no forehead brace. The headgear is quite stable and comfortable. The material the thin cushion cover is made of is quite "clingy" to our face helping to reduce leaks. And I can side sleep either side as well as my back. Even tummy sleep (I'm not one to bury my face in the pillow. I turn my head to one side or the other). The full face mask diffuses the air pressure so that it is not blowing so directly into the nostrils causing the chapping inside our nostrils.

And talkaboutsleep.com is offering FREE return (pay shipping only if I remember right) of a mask that doesn't work out for you, up to 2 masks a month. Of course they don't take insurance, you have to pay out of pocket - but you only pay for one mask no matter how many it takes to find THE right one.

Fred keep in mind that it is the EPAP which keeps Obstructive events at bay and the IPAP which prevents Hypops, snores, and such. The point is dropping EPAP for comfort over the long haul may not be productive tho it can help one get accustomed to breathing with some pressure blowing in your face.

I mention this only so you will know if she complains she doesn't feel quite right the next day. Also if you see an increase in the AHI you'll know why. You have to find that happy balance between comfort and function.

Thanks GumbyCT .....

I'm aware of that ... I will be downloading detailed reports to my PC everyday and taking a "REAL GOOD LOOK" at them. We're also setting in motion a new sleep study/titration .....

I just got up to get ready for work. I asked my wife how it's been so far ..... she said GREAT .... looks like we may have to kick back the humidity a little but I don't think she's slept this good since she got her machine.

I'm keeping my fingers crossed that everything looks good when I check it out later. Her OSA was "MILD" so I'm hoping that it doesn't take too much to keep that AHI down .... it's been averaging under 1.0 consistently

Whelp the stats look real good .......

Pressure ran most of the night at 10.0 .... there were two places where it went to 13.0 and 13.4 and then came back down

Leaks were 0 except for three small spikes .....

Apnea index: 0.0
Hypopnea Index: 0.3
AHI: 0.3
% time in Apnea: 0.0

Biggest problem was rainout .... I ordered a heated hose from Australia

AND .... she slept great

Looks like we may be onto something here ...... I'll keep checking and keep posting with what I find

Just wanted to let everyone know where we are at .....

NO MORE NOSE BURNING ..... UNREAL, HIP, HIP, HOORAY

Terry is slowly increasing her # of nightly hours .... she's averaging around 6 1/2 hours per night now .... UP FROM 4 !!!!

She's only had 1 Apnea over the past 4-5 days .... her Hypopnea Index has been down around .3 .... except one night it was .9

Leaks are pretty much non-existent ...

as previously mentioned her Min EPAP is set to 6.0 .... her Max IPAP is set to 16.0 .... her Pressure Support is set to 4.0

With these settings her pressure runs 95% of the time at 10.0 (6.0 + 4.0) .... the other times it goes up slightly .... never aboue 11.8 so far ...

Things are looking great ... she's feeling great. We are still going to setup another sleep study now that she is out of AFIB ...

I'd like to say thanks to everyone for all of their help in helping me learn what I needed to know to ease our situation. If it were not for all of you I suspect Terry would have continued using her CPAP less and less ... and who knows where that may have lead us.

Thats Great Fred, I have been following Terries progress and you have done a good job,
The default Pressure support is 4, in your case you might try bumping it up to 5 to get to the min of 11 per your wifes perscription (6 Min EPAP +5 Pressure Support) in the Vauto mode
One more thing I adjusted for my comfort was the
Ti Max: 2.0s ..I bumped up to 3.0s
It helped me when I needed to take a good long deep breath every now and again! That is just a Max setting, most breaths in are much shorter.
My prescription is 12 IPAP over 6 EPAP so I had to set VAUTO mode to EPAP 6 IPAP 12 with Pressure Support 6
I didn't like using the S- Spontaneous mode at all.
