S9 Newbie - sleep analysis

Hi Folks, recently diagnosed with sleep apnoea, I am on my second week with an S9 Elite. At time of issue I was told AH 10.0 was good and during my first week an average AH 5.0 achieved. However, I am now getting readings of AH 30+ and back to restless sleep and feeling lethargic again. What am is going wrong?

The good thing is that you have an S9 Elite so you can get data and see what is happening.

There is a member, Uncle_Bob, that has ResScan software available in his signature. You can download it onto your pc. You should have a SD card, like the ones used in cameras, in the back of you machine. If not, pick one up.

Without data it is difficult to say what is happening. My first guess is a high leak rate, but that is a guess at best.

Don't give up, eveything will be fine.

Please register and fill in the equipment section in text, not pictures. Then do a search, if you haven't already, on downloading and using Resscan. Then do a search on how to post copies of the graphs from Resscan.

Until you can post a lot more definitive information trying to answer your question is like using a BB gun to shoot a duck flying in the fog. In other words, we can do a lot of shooting and make a lot of noise but we aren't going to hit a d*** thing except by accident.

OTOH, if you do post detailed data, you'll probably get some very specific suggestions.

The basic choices are
A) Pressure is too low.
B) Pressure is too high.
C) Pressure is just fine -- your AHI is up becuase you are awake for some other reason.
D) Leak rate is too high.
E) You seem to have one of the more exotic forms of SDB and should consult your sleep doc again.

But without data, you might as well ask a magic 8 ball what's going on with your therapy. It would be as likely to give you the right answer as we would. If ResScan gives you any trouble, post and someone will help you.

Have you reported this to the sleep doctor's office? If I were you, that's the very FIRST thing that I'd do. And I'd push to be seen by the doc, a PA, or a nurse practitioner as soon as possible---within a day or two at most. And bring the machine or its SD card with you. And ask the receptionist, nurse, or whoever is responsible for doing the data download to be sure to download ALL the data and make a printout of ALL the downloaded data---not just the compliance data.

I'd also get the software from Uncle_Bob and start looking at what was different between Week 1 and Week 2. And based on what I saw, I'd take some appropriate steps about addressing the problem.

Leaks? Maybe. In this case, you need to start working on fixing them. Post your mask and the suspected problems and you'll get plenty fo tips.

EPR setting affecting AHI? Maybe. EPR is supposed to be a comfort feature. But some people find that if EPR = 3, the amount of time that they spend at the "exhale relief pressure"---which is fully 3 cm below the prescribed CPAP pressure---may be low enough to NOT provide effective therapy. So check the EPR setting and see if you can live with EPR = 0 or 1 instead of EPR = 2 or 3.

Lots of centrals showing up in the data? That's definitely a question for the doc. Did centrals show up on the titration study? Were centrals not there during week 1, but now are? If so, why? All good questions for the doc.

Possible problem with titrated pressure? Maybe---particularly if the distribution of events is similar to those on the diagnostic study in terms of AHI, the breakdown between OAI, CAI, and HI. Again, something for your doctor to address in my humble opinion.

But I myself would be cautious at only two weeks into therapy about making drastic changes to the pressure settings all by myself.
