Why do you conclude that this is how "most" go about attempting to lose weight? My n of 1 is following Weight Watchers/DASH meticulously, which is far from faddish. And the people I know who are losing weight are also all WW people. Won't work because one has to make the meal first and if I am hungry I just scrounge in the fridge. My mother (no longer alive) was a WW lifetime member and leader. I have nothing but respect for what WW does. A healthy lifestyle with proper diet and exercise that can be maintained for a lifetime should certainly be the goal for all humans of all weight, and I know of no healthier approach to the matter than WW. That said, the results, when measured, longterm, can be disappointing for helping the truly obese as compared to the merely somewhat overweight, as I understand it:

Someone with moderate-to-severe OSA who is obese needs a firstline treatment for OSA beyond Weight Watchers, in my opinion.
My point in listing the different extremes people take to lose weight is to point out that not all forms of weight loss are equal and that common approaches are not always healthy. That point, in my opinion, is a message that too often gets lost in the rhetoric.
I'm no expert, though. Just opinionated--thus my hyperbole in listing the approach of "most." Thanks for pointing that out.
I need to lose some weight myself. WW would probably pay me not to mention my mom's affiliation with them.

I don't think we're disagreeing. I'm not sure I'm necessarily disagreeing with SU, either. I would think CPAP should always be the immediate treatment. I think the question is whether CPAP in those who are obese at diagnosis can ever be discontinued after weight loss. Lifetime WW member here (since 2004). In retrospect I wonder if my weight drifting up in the last couple of years was due to onset of apnea, but there's no way to tell.
