FYI -- I contacted istby2 about the issue of intering one's email address in this survey and got the following response:
The issue of leaving an email address is entirely up to you. If you don't want to leave REAL email address - just put in something like: ihavenoemail@anywhere.com
Presumably this means use only letters and no numbers. But apparently it is ok to use a nonexistent email address name, if you really wish to try the survery. And you can view this person's website if you enter the same website address but without the "survey" at the end.

Confused, why would anyone want to voluntarily give out
this much info when it appears to be someone trying to profit off us apnea sufferers? And doing this on cpap.com's forum! If he wants do make money advertise like Johnny does, instead of riding off CPAP.COM's dime. Seems tacky IMHO.
