Ventilator Changes - The Latest Update

Well, it looks like I will be switching to SIMV Mode on the ventilator, hopefully tonight. After yet another night last night of being woken up every hour to high pressure, I'm hoping this mode works better for me.

I am certainly wishing you luck and lots of it. You must be exhausted by the frequent awakenings you have been experiencing. Here's to some well deserved restful nights ahead.

SIMV Mode -- Synchronized Intermittent Mandatory Ventilation. But I've already run into a snag since there is plain SIMV Mode and PC-SIMV Mode. I think he meant PC-SIMV because I need to set it to that to get the other options he mentioned. I've sent him an email hoping to confirm and get the correct setting for one other Inhale Pressure option. If I don't hear from him tonight, I'll try one setting (10) and if that doesn't feel right, I'll set it to 14 which is what I'm used to.

Thank you, Jeanette! Exhausted doesn't even cover it. I really hope this works better for me. AVAPS was good for a while, but obviously not doing really well for me in the last few weeks!!

Are you talking about a real VANTILATOR, or any of these?

PR System One Pro costs around $850 (this is the standard full data CPAP machine)
PR System One Auto costs around $900 (this is the standard full data APAP machine)
PR System One BiPAP Pro costs around $1900 (this is the standard fixed bi-level PAP machine)
PR System One BiPAP Auto costs around $2100 (this is the standard auto bi-level PAP machine)
PR System One BiPAP S/T costs around $5200 (this is the minimal version of the servo-ventilator machine)
PR System One BiPAP AVAPS costs $6000 (this is the fuller version of the adapto servo-ventilator machine)

It's a real ventilator, Respironics Trilogy 100. My situation is complicated and unusual for this forum.

Once again, this RT has proven worth his weight. He got my email and called me around 7:40 because we do NOT want PC-SIMV. The options ARE there under the SIMV mode, but there was another option that confused me so I missed the others.

I'm all set now and ready to test drive this for a while before going to sleep. I'll report in tomorrow about how it went.

Thank you! The more fingers we have crossed, the better! Yes, this RT is definitely good. Too bad he's not permanent, but I'll take what I can get. What a refreshing change from the previous RT!!

Lets hope you are not be ready for PC (Pressure Control)-SIMV just yet. Wouldn't PC-SIMV be the last option left on your Vent?

My RT called me and said that we definitely wanted regular SIMV mode, which is what I set it to.

I don't know about PC-SIMV being the last option, but it matters not as I didn't get out of the starting gate last night. My "test drive" lasted all of about 5 minutes as the "Low Inspiratory Pressure" alarm went off CONSTANTLY. And I do mean constantly. I tried adjusting some of the settings to see if I could get it to stop, but nothing I did stopped the alarm. And I'm guessing in SIMV mode, the alarm has to be enabled as it would not turn off.

The feeling of SIMV mode was weird, but I would have given it a chance and tried to see if I adjusted to it, but that alarm made it impossible to even try.

So for last night, I chose the lesser of two evils -- AVAPS that wakes me up about once an hour or SIMV that makes an alarm go off every 5 to 10 seconds. I must have been really tired last night because I only woke up about 4 times from 11:30 until 6:00 am.

I don't know what to do now. I will obviously contact the RT and let him know. But I really starting to hate this.

That is too bad, Madalot. Hopefully, it will be an easy fix for your RT, and you'll be able to really give SIMV a full night's test run.
