do you agree that from this starting point, if you are still tired with no leak problem, that it is only one direction: which is taking the pressure up gradually for few points.

I was titrated at 10 cmH2O but found my recovery at 13 cmH2O.
although I am faced with other set of problems from the higher pressure but this is a different subject

I am new to this and need your help please
I have been on CPAP for 2 months and I am still tired and can't function
I even raised the pressure on my machine several times and didn't help. I have no leak to speak of because I have data capable machine. the AHI comes between 2-5, yet even with best AHI of 2, I am still tired

any thoughts.???

Williamco, I too was curious regarding your responses to the advice given here by the "best and the brightest". I know these pros to be kind and trustworthy in a straightforward and sensible way, only wanting successful therapy for us. Even now after a year I'm still amazed at their wealth of knowledge on this complex subject of OSA. I can remember the only time I didn't heed their rules and changed several things in the same week, straight pressure to auto, Aflex to Cflex and the addition of a new bed pillow. Simple stuff I thought, but boy, we were in a mess for a few weeks, didn't know which way was up! I received a few gentle 2x4's for that but the scars have healed by now. Good luck, williamco.

the first date in first quote was the date I JOINED "Joined: 15 Dec 2008" not the date that statement was written

I joined much earlier as I took a long time to convince myself and get used to using CPAP.

selfdiagnosed and selftreated because I left my official titrated number and decided to raise the pressure on my own in persue of recovery that never came from sleep study. Also sleep study was done last year but treatment on my own started June 17 this year so that study is not part of my campain now

Here we go again, the same crap I left apneasupport.org for, people there are strange thinking that others are liers, trying to promote their own agenda, have commercial interest to sell some products, are not really apnea patients, have to have proof of diagnosis, no selftreatment is allowed, STUPID accusation flying everywhere...etc

Well, I was looking at it more from the point of figuring out the issues. I believe it comes down to two possibilities, and then the combinations, namely, you have sleep apnea and/or you have one or more other issues. Those possibilities being:

You have sleep apnea and other issues, both under control.
You have sleep apnea and other issues, sleep apnea under control.
You have sleep apnea and other issues, other issues under control.
You have sleep apnea and other issues, neither under control.

The first step would be to determine if you, in fact, have sleep apnea. You told ozij that you were self-diagnosed, which would make a correct diagnosis suspect. If you in fact had a sleep study, then we could more reliably say the diagnosis is correct.

If you've only been on therapy 2 months, then that may be too soon to expect results in re: OSA treatment. If, as the posts on the other forum may have suggested, you were on therapy for 10 months, then the likelihood of the secondary issues would come more into play and deserve the more aggressive attention.

basic facts - maybe it is all there -

you had an in lab titration and they determined the optimal pressure to be 10 ?

when was that?

you decided to play with your settings (as many of us have done) and in doing so you first came up with a better optimal level of 13?

you decided to play some more with your setting and have raised them more?

you discovered things you though were "cures" really weren't along the way

are you seeing an increase in AHI as you raise pressure? I sure do myself

you have stated leaks are under control ------- great ----- can you post some sample nights for events - perhaps the timing of events can help some the "experts" here see what is going on ---

When this starts happening you have to be suspicious that you are infact inducing centrals. Remember that to most CPAP machines centrals will be scored as obstructives.

the facts as long as old posts have surfaced up (but I learned I should have full complete history from now on as conscise as poosible ):

1- yes I had been titrated at 10 but that was last year in Dec (only OSA)
2- I couldn't bring myself to get used to CPAP except in June 17, my first night was only couple of hours-pressure 10-14 - the fact that my study was 7 months before starting treatment made me suspecious about being relative to my case now
3- couldn't feel any better for a month- also AHI was 7-8 - machine spent more time on the upper limit
4- I started to raise the pressure on 3-4 days interval
started to feel better when minimum pressure was around 15- AHI was 2.5-3 - pressure Auto 15-17 but machine then didn't touch 17 and spent 15% of the night at 16 and the rest at 15

5-A month later which last 2 week: machine started to spend more time at 17 (35 %), AHI started to creep up till 5.5
6- last night I raised the pressure 16-18 but took the mask off after couple of hours, because I had nasal pain from the pressure. AHI was 1.7 but machine again spent too much time at 18 (30%)
7- I want to try CPAP today at 16 as suggested by many of you
8- I want to collect samples of my report that represent every step of my case (5-6) and post them here if you bare with me

Can't remember if you've addressed this, william, but are you mouth breathing/leaking with that nasal mask?
DeVilbiss IntelliPap Std Plus with Smartflex; Transcend miniCPAP & Everest2 w/humidifier & batt for travel. UltraMirage FFM; PadACheeks; PaPillow. Using straight CPAP at 13.0/passover humidifier. AHI consistently < 1.5. Began CPAP 9/4/08.

I believe he said he used an oracle with the holes plugged to block the mouth but he hasn't posted any leak graphs - just says the leaks are fine with this set up.

Again, if I am wrong, please correct me.

yes jules you are exactly right. Nasal mask, with Oracle mask in mouth with its holes plugged up to prevent mouth leak
my average leak with this system is 35-38 LPM and the intentional leak for my mask under this pressure is 35 LPM
