Am I Interpreting the Report data correctly?

Post by ChipsXJ » Wed Mar 04, 2009 8:35 am

Am I Interpreting this correctly?

Obstructive Apnea Event = the average number of apneas per hour over the complete time period.
Average Vibratory Snore Index = the average number of time the machine detects a snore (even if it is not really a snore) over the complete time period.
Leak: (I really do not feel I understand any of the leak data very well)
Average Max Leak= (not sure about this) my average large leak over the period (114.2 cmh2o per day)
Average 90% Leak = the average leak rate I was at for 90% of the time( 57.0 cmh20 for 90% of the time)
Average Leak = the average leak rate over the period of time ( my average leak is 49.8 cmh2o)
Average Large Leak = the number of minutes I was in large leak over the period. (average of 17.7 mins at a time)

Last Nights Report:

It appears to me to read like this:
OA: 2.2 or 2.2 obstructions per hour averaged
H: 0.7hypopneas per hour averaged
VS: 18.7 instances the machine thinks I snore (hose rubs or something or actual snoring)
Large Leak
0.0 MIns: no large leaks (not sure what constitutes a Large leak) for any minutes.
0.0% of Night: no large leaks for any % of the night.
Average Leak: 44.41 cmh20 average leak for the entire night.

Overall what do you guys and gals think? how does the the leak rate look?

This last night is with my new pillow friend (body pillow)to 'try' and stay on my side and my new f&K 431 mask.

You have a good understanding of the data that your report gives.

I'm looking at the "Sleep therapy Daily Details" page (page 4). I note that the leak rate for a Flexfit 431, at a pressure of 10 cm should be 40 (the graph that shows this is on the instruction sheet that came with your mask). That is the vent rate and it is the amount of leak your machine will compensate for at a pressure of 10 cm. Your true leak rate is then, 44.41-40=4.41. That is a little high. I note, on your leak line, some peaks. There seem to be events corresponding to leaks, at several points (if you follow the vertical dotted line up you can see that). Those events may really be in response to leaks.

While your leak rate is not too bad, there is room for improvement. You may have the mask too loose or too tight. It's a matter of trial and error. It also could be that your mask is moving (and leaking) when you turn because the hose pulls on it. A hose management setup could help here. Also, I use the "Performance white 6' hose". It is so much lighter than the one that comes with your machine and there is less chance of it pulling your mask and causing leaks when you move in your sleep.

All those Vibratory snores could be your hose hitting something and, as you said, maybe not snores. A hose management setup and the lighter hose I mentioned above could help this too.

It could be that your pressure needs to be a little higher but I would work on the leak line first trying some of the things I suggested above.

You AHI (sum of OA - Obsrtuctive APneas and H - Hypoapneas) is quite good at 3 as is your Long Term average AHI of 3.9.

Your "Sleep Therapy Long Term Trend" shows much the same leak data except that you also some Large Leaks in that period of time and, on some nights, your leak rate was much higher that the "Daily Data" page showed. These can worked on with the same suggestions I made for the "Daily Detail" page. It looks like, on this Long Term Trend page that you are beginning to have fewer OAs and Hs as time goes on so there is pretty good progress. VS is a little irratic.

You are doing quite well. In my opinion, the suggestions I made in this post could make it better.

My thoughts anyway!

Thank you Hawthorne.
Last night was my first night with the new mask (only used my first mask, mojo, for 30 days or so) so I know I will have some tweaking there.

A hose setup is on my list of projects for the weekend, sooner if I have time.

Thank you again for your input and guidance on the charts.

I never even glance at the Long Term Trend, or any of the report pages other than the "Daily Details" pages that begin on page 4... starting with the Daily Details graph you included as the second picture in your post, Chips. Glad you posted that page.

Considering the blocks of Obstructive apneas showing up in the latter part of the night (very likely when you were in your longest REM sleep) if most of your individual nights look pretty much like that (OA's during last half) I'd probably raise the pressure to 11, or maybe even 12, if it were me...and only if raising the pressure a cm or two didn't cause mask problems. Your overall AHI is very low, but if apneas are happening mostly in the last part of the night for you, I'd wonder if they might be interrupting REM then...and I'd want to use a bit more pressure.

Doesn't look like you're having any significant leak problem. The big spike in the leak line at the beginning of the second half of the night looks like you might have started the machine before putting on the mask, or took the mask off (perhaps to readjust it) and then put it back on. That spike probably skewed your "average leak" number. Taking that into consideration, I think your leak rate looks ok.

While it's very possible for things other than true snores to get marked in the "Vibratory Snores", the sheer number of "Vibratory Snores" you're racking up during the night probably are "snores." They might not even be audible to a bed partner...might be just a soft flutter of the soft palate. Raising the pressure should clear those up, too, for the most part.

All in all, even if you don't change a thing, your data looks good for that night.

RG, never really noted when most of hubby's OAs happened until now, was surprised to see that they do occur mostly towards the last few hrs of the night. I still find it difficult to interpret the Daily Details. I kind of know that he should probably be back up to min=12 but I've been hesitant to change anything since I've finally gotten his leaks under control more or less with a new mask(same UMFF). I guess I could try it and if he starts leaking too much, put it back down to 11. His AHIs have been good, about 1.0-1.5, 90%=12, and he's still snoring a bit too. Am I on the right track?

I very rarely look at the "Long Term" trends either but, in this report I found it interesting. I noted that, over the number of days ( it looks like 14+ days) reported on the Long Term trend page, the Apneas and Hypoapneas really stabilized at a very good level after some initial quite high numbers.

The Long Term trend page also showed quite a bit of "Large Leak" and some quite high leak numbers with a very high "Average 90% leak" over that "Long Term" period of time that do not show on the "Daily Data" page for the 1 night.

Those factors made me wonder about hose management. I was thinking hose management and a small rise in pressure, perhaps after the hose management is in place, could eliminate a lot of the Vibratory Snores.
