Newbie - HELP (327.20)

Post by tsheff1 » Wed Jan 21, 2009 2:35 am

Hello I'm new and would like to ask about my sleep study. I'm scheduled for a follow-up CPAP titration trial in less than a week. For the last 5? years I lost all energy and gained too much weight(used to work 60 nhrs a week plus work out 36 hrs a week). Blood tests done but never a sleep study until my cousin told me to TELL my doctor to send me for one! If this works I'll be so happy but still upset no doctor could have figured this out earlier! When I was working out back then I was diagnosed with exercise induced asthma but on my report I received today it sais I have bradycardia which sounds like it may be the major problem! I also have a severe deviated septum(never broke my nose!).
In bed 500 min and slept 432 yielding good sleep efficiency of 87%. Sleep latency 7 min., REM latency 103 min, light NREM moderately restricted at 22min, REM sleep was within normal limits at 82 min. Sleep architecture was severe disrupted with 256 stage changes including 79 to transitional N1 sleep across the night
Mean continuos oxygen saturation during wakefullness was 97% with minimal desaturation of 93% observed.
0 apnea.1 hypopnea of 14 seconds. .1 events of AHI per hour of sleep, in normal range.
EKG detected bradycardia
Sleep moderately fragmented primarily due to snoring and spontaneous arousals with a total of 152 transient arousals yielding an index of about 21 per hour.
Study ducuments the presence of moderatelty fragmented sleep primarily on the basis of increased upper airway resistance and minimal oxygen desaturation. Advised to follow-up for CPAP titration tial. She may also wish to pursue upper airway evaluation for alternative treatments.
My question - the doc did not even mention bradycardia but all my symptoms seem to reflect that(bradycardia). Bradycardia is good for an athlete but in my case it does not sound good! Before I came in fror my sleep test the doc tried putting a 'scope'?? up my nose but couldn't because it was too narrow. I don't really know anything about this but what I've found searching the web points to sinus bradycardia.
Please may I have some opinions? I do understand I am not to perceive opinions as medical advice! Anyone else have any issues like mine? It appears my sleep problem isn't bad but it's terrible! Naps make me more tired (I can't even take them as they makeme more tired) and sleeping seems to give me no rest at all. I'm at a point where I don't like sleeping because it makes me more tired! I sometimes have issues driving - fighting to keep myt eyes open! (they even wrote if I'm tired do not drive- but it just happens)

I think you need to take this one step at a time and, with your doctor, assess as you go. You are ready for step 2, your titration sleep study. That will determine a pressure for you and then you will probably move on to cpap therapy.

It may take a while to see results from that, but gradually, you should feel more rested and not feel so sleepy when driving.

Keep us posted. We've all been there and there a lots of people on this forum able and willing to help you.

Let us know how the titration goes.

Just a suggestion, but have you had your thyroid checked - the bradycardia could be related.

I've since had a UPPP but, my brain will not stay in deep sleep. I now work at a pulmonary office and we have a sleep lab in the back. One dr suggested possible narcolepsy and as I look about that, seems I have all symptoms. Will be doing a 3rd study. After my UPPP id had a 2nd study. No apnea but terrible sleep. I cannot find my report so will be calling for it but, they basically said every time my brain hits deep sleep it wakes itself up so... We shall see... This has been going on far too long and although I'd never thought it was narcolepsy, brought it to forefront. Going to eventually get MLST

Are you currently using any form of CPAP/APAP/BiPAP? (referred to as XPAP here)
From all reports I've read, UPPP is only temporary help and the patient eventually ends up back on XPAP therapy of some sort.
