self tritration result

Post by Ms Piggy » Sat Jan 03, 2009 3:49 pm

Hi, last night a friend of mine used my machine set at 4 to test if he has a problem. We thought he had to some degree as his blood pressure is a bit on the high side, he sleeps a lot but is tired, he thinks his father had undiagnosed SA.
apneas 2
centrals 7
hypops 22
" flow 1 total 32 divided by 7.5
giving a the result is an AHI of 4.266

although this IS under 5 I thought that in view of the above he should maybe ask his Dr. and perhaps get a referral for clinic testing. any thoughts please. Should we give it another night?
thank you.

It isn't just how many events, but how long each event was. If he has insurance or can afford to pay for one, he should have a sleep study done. With centrals you need to take extra care with treatment.

No need to be sorry. CArdiac Oscillations are the 420E's way of identifying central apneas. Heartbeats can be sensed through an open airway, but not through an obstructed airway. Heartbeats (cardiac oscillations) can be picked up by the 420E, and those "cardiac oscillations" combined with lack of airflow mean it's a central apnea.

Your friend really should get a sleep study.

A pressure of 4 isn't much, but it's some. So, if he was having some events (like the 22 hypopneas) at pressure of 4, it's possible he could have quite a few more when sleeping the way he usually does, with no cpap at all.

My friend is going to see his Dr. for a referral for testing but in the meantime I thought it might beneficial if he kept up with some therapy (preferably on my basic Remstar as I will need the 420 to help me evaluate a test drive mask soon) as this could be another month or most likely longer.
we tried another night on my pb420E setting the pressure at 5 - 20 with Fl2 only On.
His first 3 1/2 hrs showed no events, but a few runs, not surprising maybe as he slept on his side/stomach. (in the Swastika position).
pressure registered 4 the whole time.

the second 3 1/2 hrs were not so good, as he slept on his back - he has neck pains if he sleeps on his side which he claims was caused by a chiropractor's manipulation some time ago, ex rays looked ok, some sign of arthritis. it's the figures I am concerned with here.
result
pressure registered from 4 to 7.
2 apneas
3 " CA
14 Hypops
l " w/flow
Maybe something in the setting could be improved?
Any suggestions or comments would be appreciated, thank you. and thank you for the previous ones.
