Yes, the doc said there were some centrals at my last psg. I'm not sure if they happened at 12 or 14.
But if it don't work increasing the pressure then you might as well lower it, lower it to 9 if AHI falls you know the AI you have been looking at is central not obstructive. The HI on that machine doesn't mean anything.
And No, that machine cannot tell the difference between obstructive/central either, all it sees is 20 to 40 seconds of no airflow thinks its an apnea when it can be just as easily a central. Despite what RG says, a central can take you back to a wake state, it can be seen on any PSG Sleep Staging graph, if you have your first PSG results check it out. Take a ruler where those centrals are seen and go up/down to the Sleep Staging graph and observe what stage of sleep you are in when they hit. I'm not saying all CA's take you back to a wake state but many do and it is not the conscious wake where you know it.
With centrals even a few if you increase pressure (like they did with 14 cm) you will feel much worse the next day. You either have to blow through them with pressure or stay below where they show up.
I would try going back to 8.0 cm get your AI, go to 9.0 cm, get your AI and compare to current 10 cm AI. When you are at the pressure resulting in the best sleep and fewest of both obstructive and central events the AI will be the lowest. IGNORE any HI on that machine it is meaningless, go only by the AI and leak.
