I have a new Mako 3500 reader that has been used twice. My doctor is swapping my machine for a ResMed, so I can no longer use the reader. If anyone is interested in purchasing the unit, send me a PM. I also have the software, EncorePro 1.8i. Let me know

Was told that the ResMed will address my RERAa where the respironics won't. Is this true?

Not true. Neither machine (nor any treatment machine) can spot "RERAs" (Respiratory Related Effort Arousals) because the machines we use at home cannot "see" arousals.Jcochran wrote:Was told that the ResMed will address my RERAa where the respironics won't. Is this true?
RERAs are seen during a PSG sleep study. The wires on a person's head (providing EEG data) and the belts that were placed around your chest and abdomen (to measure respiratory effort...the heaving of the chest and abdomen trying to pull in breath past a collapsed or partially collapsed airway) are how RERAs are seen and noted.

RestedGal, Thank you so much for all your knowledgeable answers. One question, just to straighten something out for me. If the RERAs are noted during a PSG by the EEG and Abdominal belt, then why did my original PSG show nothing? It wasn't until a nasal sensor was added and the PSG repeated that the events were seen. Any ideas what the nasal sensor was? Was told it monitored air intake. Also, if the RERAs were not seen until the nasal sensor was added, then how could a CPAP titration test be performed without the nasal sensor that saw the problems being used? Thanks

While I agree that no machine can respond to arousals, as Rested Gal has pointed out, I think there may be something valid (theoretically) in your doc's decision to change your machine.
The way I understand it addressing RERA does not necessarily mean responding to the arousals, but rather to the breathing disturbances that caused them. Resmed's are more aggressive (i.e. quick) in responding to preliminary indicators of obstruction - so even an Autoset, set on automatic mode, may treat your type of breathing better than another machine. This would not be true if the doc. suggests fixed pressure on any CPAP. And, if you are being switched to an SV machine, that machine will respond to the waning of your breathing - which may again be related to your RERAs.
The bottom line is that in order for you to get reliable info from us on this forum, you have to give us very specific data: Your PSG reports if possible, and the exact name and pressure setup details of the machine0(s) your doc. proposes.
Do your best to have the machine /machines on trial until you know for sure what you diagnosis is, and what you results are on each type of machine.

I'll have to scan my original sleep reports. Will do that tomorrow. It just seems to me that the nasal sensor would have to be in place to properly do a CPAP titration. Anyway. I have read on here that the Respironics does not respond to one event, rather a cluster of events. I've also been told that once a respironics does their famous "runaway" it takes like 45 min. for them to come back down. That was my original problem, The machine on APAP was waking me up and I couldn't get it to stop. Yeah, I could press the ramp button, but that defeats the purpose if I keep waking up every two hours. The machine ran at its max pressure, no matter what the max pressure was set at. I may be just grabbing at straws here, but I told my doctor/DME that we had to try something new because the current treatment is not working. I was prescribed provigil to at least try and make me function, tomorrow will be the first day of use. My DME, which could be not telling the whole truth, said she called Respironics and they told her that the machine I have will not address my symptoms. She then called ResMed and theirs will. I have no idea, I just want to feel normal in the mornings and be able to go one day without having to take 10 naps.

Are you going to two doctors? Was it the DME or the docotor who suggested a machine switch? Is this an accredited sleep lab? I'm so confused that for a minute I thought you were pulling our collective leg (don't take that presonally).Jcochran wrote:Hello Gang. I asked the doctor what to do for until then and he said he'd write me a script for provigil, then find out that he has to give a pre-approval before insurance will pay, which is over $400, and that could take 5-7 days.
Could it be time for you to look for another - accredited - sleep lab?What in the world do I do???
