Perhaps this will help. While you are paying attention to your breath going in and out, try associating it with images. For example, taking a walk through the woods, or on a beach. You're breathing in and out while walking in a place you really enjoy. This type of imaging may help relax you enough that you'll forget to pay attention to your breaths, and before you know it, you're asleep.

Concerning the noise, I placed the machine on a towel, and moved it from my night stand to the floor. This made a HUGE difference, and now the sound is at an acceptable level for both me and my wife.
Night 5 was good: I actually kept the mask on until 5 AM: that's the longest I've lasted. I slept fitfully, but still seemed a bit more alert.
Night 6 (last night) was a step back. I got really frustrated and took the mask off about 1 AM. I just wanted a "normal" night of sleep.
This morning, I am exhausted. The fact is that my "normal" night of sleep is way worst than a fitful night with the mask. It is clear there is no going back for me: I have to learn to deal with the mask because it is my only hope to getting better.
Finally, when they setup my machine, I watched what the guy did to get into the clinic menu. I looked at it last night and there is some efficacy data. Once I get a good night logged, I'll post the results here for help deciphering it.
