I have received several emails from SRC_US_Post Market, with the email address of Soumyadip.Bhunia at philips.com. It reads:
"Hello,
I am with Philips Post Market Surveillance Good Faith Effort Team. As part of Philips Respironics Complaint Handling Process, it is our obligation to report adverse events to the Regulatory Reporting Agencies. We are asking you to verify the serial number and model number of your device located on the bottom of your machine. Please refer to the attached document on how to locate the serial number for your device. To ensure that we have accurate information for the complaint, we would like to request additional information. When responding, please include the above notification# in your reply and click “reply all”.
Please provide responses for the following:
• Did an alarm sound at the time of the event?
• Was this is an out of box failure?
• What is the serial number of the device?
• Do you have any additional information about the experience with the device?
Thank you in advance for your prompt response. This will ensure the complaint to be processed in a timely matter to better serve you."
I ignored the first couple of emails, but finally responded to the last one saying that I did not file an "adverse event" report. Your thoughts?
