Hi Everyone,

I'm hoping to get some assistance with titrating my Bipap to treat my UARS.

Background: I've been struggling to get restful sleep for almost six years. My most recent sleep study (18 months ago) titrated me to 14/9 on Bipap. I felt great leaving the sleep lab, even though I had only gotten a few hours of deep restorative sleep.

When I got home and used this pressure (on a Phillips Bipap), I experienced an incredible number of CAs, felt awful, and I eventually retreated to my ResMed Air Sense 10 Apap with limited success.

I also have PLMD which is correlated with my sleep-disordered breathing ("SDB") and creates its own set of arousals. Per my sleep study, it appears that my PLMD resolves when my SDB resolves.

In addition to my new ResMed V-Auto Bipap I am now using, I also use an oral appliance and take .25mg of clonazepam at bedtime. This combination gets me to a point where I can function, but I am miserable and unrested. Obviously, I'd like to find a purely mechanical solution to my SDB and discontinue clonazepam. I'm also open to getting a different machine, up to and including an ASV.

It seems that I am stuck in a permanent game of tug-of-war: If I raise the pressure I need to a point that is adequate (based on in-lab titration, i.e., 14/9), I experience a high degree of centrals, if I keep the pressure low, I experience obstructive events, predominantly in the form of RERAS and to a lesser degree, hypo, and full-blown apneas.

Welcome! There are two things you could do to help us help you.

First, put the information about your mask and machine into your signature block. That way it'll be there whenever anyone reads any of your posts.

Second, if you have a laptop or desktop computer, download the Oscar software. You'll need an SD card in your machine and a way for your computer to read the card. Does that sound do-able? We'd be happy to help you with the process if you have questions about it.

The reason for using Oscar is that it provides a wealth of data that will give people here a basis for making recommendations about your settings.

It's a "bilevel" or "bpap" perhaps, but not a Bipap, just like Ford doesn't make a Corvette.

Resmed's bilevel algorithm differs enough from respironics' "bipap"
that they are not completely interchangeable. [[[You got the good one.]]]
Some day, someone might try to pull a fast one on you,
and you might not be happy with an actual "bipap".
Cool name--not so cool everything else.
