Centrals up and on asv

My hb's sleep test results.: ahi 53. We started cpap and then bipap and now it has been a week on asv. However nos are still high. Yesterday, for the first time his spo2 stayed generally above 90%. Usually it drops till 60% and then back up and then down and up again. However his ahi shot up to 53 which is generally Centrals. I was told that I need to control my leaks so that the Centrals will go down. Is it true?
He is using a bmc mask. I have really tried but I couldn't find the leak. The only place that has air coming out is the exhaust ports which has a constant air flow blowing out.

My hb is a stroke patient. Appreciate any help

Side note I have also asked if the pressure set is too high for my hb cos when we first started, my hb couldn't exhale due to high pressure and it was a scary night. Was told to hang on to this settings first till I settle my leaks.,

We don't know how bad the leaks are but yes really big leaks can definitely impact the effectiveness of any cpap/bilevel/asv therapy because when big big leaks are present the pressures can't be what they need to be.

Is the mask moving causing the leaks...and you are asleep and can't find them when they are occurring?

What does the OSCAR software show as to leaks????

Yes. I downloaded Oscar. What shd I be looking at? I check my hb mask the moment he sleeps and the only constant air flow out is through the exhaust holes. I don't hear any ssh ssh sounds that is associated to leak. Even at the Ng tube area, I don't feel any leak. I am puzzled and worried too

well, one of the things you could be looking at is how to organize a chart, capture that chart image and post it here for people to look at. there is a vast amount of knowledge here, but people need to see it in order to form an opinion and give advice.

it's a lot to go through, but it's very helpful in the end. give it a try and if you have questions, ask. folk will be happy to help.

Your husband has a Philips Respironics DreamStation ASV if I remember correctly.

These leak numbers are far from horrible numbers because of how Respironics reports leaks. I don't think the high AHI is from excessive leaks.

What is the AHI composed of?

How much is the Clear Airway Index?
How much is the Obstructive Index?
How much is the Hyponea index?
