vauto basics

Post by zonker » Tue Mar 15, 2022 5:57 pm

can someone point me to a source for the ins and outs of my aircurve 10 vauto bilevel?

yes, i know i can ask questions here at any time to have something cleared up. but i'm so new to the vauto, i wouldn't even know how to phrase a question. so i'd appreciate boning up on it. either a website or a book or even youtube video would serve to help me.

thanks in advance!

You can (1) learn the present accepted overall scholarly principles of bilevel titration in the United States; OR you can (2) learn ResMed's proprietary terminology and in-lab titration protocols for their VAUTO; OR you can (3) learn some random doctor's or RRT's personal preferences and rules of thumb on how to use autobilevel (at least the five or six who actually have some idea how to do so). But to get the benefits of all three general perspectives simultaneously and in the context of how to zero in on the most effective and comfortable settings for you, I would think your best bet would be (4) to keep on asking specific questions of trusted members of patient-oriented forums and then go ahead and compile their wisdom and experience into a paper/brochure/book/website yourself.

But then again, I've become very opinionated in the days following my recent resurrection.

[14:00] Brilliant idea to use the tape measure to illustrate it!

However, I personally, as an opinionated nobody, totally disagree with his negative assessment of the beauty of keeping PS constant in auto with bilevel.

Constantly changing PS (which ResMed don't do but the other brand is often set up to do) is MUCH more likely to cause breathing instability than steady PS in which IPAP and EPAP move locked together a fixed distance away . Variations in work of breathing for someone using bilevel would be poo! (At least, it was for me when I used that other brand.)

If yer gonna do autobilevel, do ResMed's version, I say.

It is a thing of nonpoo beauty.

Lefty's got a long history of thinking Respironics is 'better', against all evidence to the contrary.

Not sure if he's decided that foamgate is a sign or not.

Well, at least he's entertaining and educational, which buys him a good bit of goodwill and slack from me, here on the online-dog-and-pony circuit.

You still being the grumpiest wise, uh, person on the internet helping people?

And thanks for anything you did to help keep Zonker alive while I was dead.

I' am a fan of Lefty, love his quirkiness, sense of humour and *most* of his information. But.....

Don't like how over-monetized his channel is. I absolutely respect content creators to monetize their channels. But with him the non-stop:
  • Youtube ads
  • Internal Sponsor
  • Mask Bright sales
  • Patreon Solicitation
  • Amazon affiliate
  • Merchandise sales
  • Solicitation for donations
  • Diagnostics business
He's the CPAP equivalent of the 700-Club.

I also disagree with some of his educational content. His anti-APAP tirades that (like many) is not based on APAP performance but misconfigured APAP performance.

This is where we have to learn how to use our own personal filters and tune out the ads.

He does have his own opinions and I disagree with a lot of them.

We have to remember he is just a regular guy who happens to be a sleep tech and he's trying to keep a forum up and running...and we get his own personal opinions about things. That's one of the prices we get to pay.
Just because they are his own personal opinion doesn't make them the gospel though....sigh. He's not going to change just because we might disagree with some of his personal opinions because after all....all we have is our own personal opinions as well.

I do wish he was a little more flexible and not so rigid with his opinions (like offer both sides) but I also wish for world peace...that ain't gonna happen either.

His website doesn't get much activity though except from spammers which I clean up every morning.

He doesn't like APAP and he doesn't like EPR which makes me wonder why he likes bilevel.
Oh well.....
