Low AHI still snoring

Post by Driver1814 » Fri Dec 18, 2020 11:07 pm

Hi all newbie here!!

Started cpap 2 month ago. Afibb doc recommended sleep
study after heart ablation. Mild SA 12 score. Struggled at 1st but have got it down kinda:)

Original pressure 5/15 resmed 10 nasal mask. I adjusted down to 4/4 until I got used to cpap, went from 2 hr a night to 7+ now wearing cpap. Pressure now 4/8.

Alright my question is I have ahi 1.4 last 30 days but I'm still snoring when on my back. Wife said I'm breathing through my nose and exhaling through my mouth when snoring. Is this common, ok should I try chin strap?? All help welcome!!

Last month report
AHI 1.5
Total AI 1.2
Central 0.8

Merry Christmas!!

Snoring is an indication that the airway is still trying to collapse. The fact that your AHI is fairly low means that those snores or trying to collapse events aren't growing up or progressing to full grown apneas.
Snores aren't part of the AHI.

You probably need a little more minimum or maximum or both pressure to better hold the airway open and prevent the airway from trying to collapse. Fairly common. The cpap is working for you with the low AHI but not optimally just yet because it isn't preventing the snores. You are close though.

I'm going to put pressure back up to the prescribed 5/15.

Had pressure 4/10 last night pressure report read 8.5. did have 16L leakage. Is 16L to high?

Get OSCAR so you know exactly what the leak is doing

OSCAR https://www.sleepfiles.com/OSCAR/
http://www.apneaboard.com/wiki/index.ph ... stallation
http://www.apneaboard.com/wiki/index.ph ... rpretation

The leak number shown on the LCD display is what we call a 95% number and all that means is that the leak was at OR BELOW that number for 95% of the night. The OR BELOW part is often not understood. People think the number is an average and it is not an average.

16 L/min 95% number is well withing the machine's ability to compensate for the leak. Large leak territory isn't until 24 L/min and even then it's more of a problem the further you go past 24 L/min. A little bit past 24 L/min isn't a big deal.

Thanks!

I also think my snoring maybe an issue with sleeping virtually flat on my back. I was given false info stating that this position is best for snoring. From what I read it's the opposite.

Any truth to beenifits of elevated sleeping position?

Supine sleeping is absolutely the worst position to be sleeping when it comes to OSA. Gravity can really make the airway tissues flop more and make for more pressure needs to hold the airway open.

So that's why we tell people to avoid back sleeping if at all possible. Some people there's a big difference between side sleeping and back sleeping...sometimes not so much difference but a big difference isn't uncommon at all.
I know a guy who needs 19 cm pressure when on his back and 9 cm when on his side....so he figured out real quickly that he would learn to love to sleep on his side.

Now I don't promote just having to stay off one's back unless there is a real need because staying on one's side all night is easier said than done sometimes. I prefer to just set the pressure settings so that the machine can auto adjust as needed and tell people to sleep in whatever position they want to sleep in and let the machine sort it out.

Since getting machine back seems to be position I find myself. Before cpap side was my spot. Looks like I'm going to be back to side at all cost:). And elevate my back position if I end up on back.
