Pitman, if I'm reading the dates on your charts correctly, you've already made changes in exactly the directions I was thinking of. You've raised your minimum -- check -- and you've turned on EPR and set it to 2 -- check -- and you've opened your max to 20 -- check.
If all of those were comfortable for you, I would recommend increasing EPR to 3, raising your minimum by 1 to compensate. Let's see if that brings your flow limitations down some.
Your obstructive events are generally clustered. There are several possible causes for clustered OAs. One is that you get a lot of OAs during your REM-stage sleep. Another is that you get them especially when you turn onto your back. And a third is that you get them from tucking your chin down toward your chest, which can create a small limitation on your airway all on its own, which turns into an OA at the drop of a hat.
If it's REM, you just need your range to be able to go high enough to deal with it. But there are simple experiments to try to see whether you can bust up the clusters without additional pressure. The easiest one is to try wearing a soft cervical collar to bed. You can pick them up at most large pharmacies. You don't have to make it tight; you just want to be sure your chin can't tuck down under the front of it.
You can also train yourself not to sleep on your back, and that's something many people have done successfully. It can take a little more time and effort, so maybe try the easier collar idea first?
Keep us posted, would you?
