DreamStation Auto BiPAP Setup

Please see my new equipment, can you please guide me on how to do the settings?
The challenge I have is the Therapy Setting Screen, what is the guideline to set the followings:
AutoB?
IPAP High
EPAP Low
PS Low
PS High

I would think that the "guideline" would have been determined by the sleep study. (if you had one)
They would have found what would be the minimum (EPAP) and maximum (IPAP) pressures required.
Many of those settings can/could also be determined by trial and error, too.

If I understand correctly, you do not have a prescription for a bilevel machine settings. You want to know good starting point settings. Someone who uses bilevel machines should be along shortly to help.

In the meantime, you might want to familiarize yourself with OSCAR software in a pinned thread near the top of the index page. OSCAR is the software of choice used to dial in optimal CPAP settings.

I am not that 'right person' ChicagoGranny refers to, but that right person is going to need SOMETHING to start with. If I understand, you're replacing another machine with this new machine. They will want to know the settings of the prior machine, at least as a starting point.

From there its going to take observation (OSCAR) and small steps...

When I switched from APAP (ResMed AS 10 AS for Her) to BiLevel (AirCurve 10 Vauto), I posted my APAPs settings and asked for equivalent settings as a starting point:

I was advised:
  • Of the equivalent EPAP, IPAP, PS settings
  • Not to change any of the BiLevel specific settings such as (Trigger, TiMin, TiMax, Cycle)
  • To consider increasing PS to 4, from my current EPR of 3, for comfort.
I followed the advice, and haven't made one change since. My comfort has increased, and my numbers improved dramatically.

I have the same machine and have only been using it for 2 nights. The AutoB default settings are as follows: IPAP: 25 CM H2O, EPAP: 4 CM H2O, PS Min: 2, PS Max 8, Bi-Flex: On 2, SmartStart: Off, Ramp: 30 Minutes, Ramp Start Pressure: 4 CM H2O. I tried this setting the first 4 hours of use, and my AHI was 6.8 which is higher than they would like. They say anything below 5 is normal but many individuals can get below 1. I changed my epap from 4 to 8 and that resulted in an AHI of 1.9. I am still tweaking mine but great to see it going in the positive direction. There are several YouTube videos that show you how to change these settings and they aren't difficult. I hope this helps you. The great thing with this machine is that you can see how you are doing every night and adjust if necessary.

Have you ever been on cpap/apap/bilevel therapy before or is this something totally new to you?

It's impossible to give any sort of guide without knowing more information.

Like past history of sleep studies or not...in lab or not?
Was a sleep study ever done using cpap to figure out pressure needs?
Past machine usage at whatever settings if there is past usage??

Without knowing any history about all we can offer is start with the machine default settings and go from there using available software to monitor results.

Totally new. Because the sleep study is using a totally different machine.
