CPAP-induced central sleep apnea?

Hi!

First, thank you so much for this website, it's a really amazing source for people who just discovered they have sleep apnea.

I've been diagnosed in October 2019 with mild sleep apnea, here's the report (Philips Alice NightOne):
  • AHI: 8.4 events / hour -> 0 central
  • Average SpO2: 95%
  • Lowest SpO2: 91%
So, I've been using my CPAP since mid October (ResMed AirSense 10 AutoSet for Her, AirFit P30i).
I'm starting to sleep a little better in my opinion, but I'm seeing since the beginning of the CPAP some central apneas. The AHI is considered as treated (less than 5), but I'm wondering what causes them, and how could I lower them. (they are all around 15-30 seconds long, and all seem the same zooming in OSCAR)

In December I tried to increase the min pressure to 7, but seems like it was the same.

Do you have any advice, or settings I can change to try to lower them? Is it real central sleep apnea, or hidden OSA, or I'm just awake a lot in the night (even if I don't remember)?

I'm including 2 nights (one with min at 7, the other with min at 6, with 1 zoom of the CA).

I notice you've got EPR set to 3.
I've been told on here that this pressure differential can cause CAs.
It might be worth trying EPR on 1 (or off) for 3 or 4 nights to see if they reduce consistently. It's worked for me, I've got a thread on here with a similar experiment (same machine too) and it worked.

The CA close up you've should appears to be when you've woken briefly. From what I've been reading here the erratic pattern just before it happens is normally awake or changing position and so that CA is not worth considering.

You really can't make a significant decrease in CAs with pressure change. I agree that your events look like slight wake ups or restless turning.

CA events on a CPAP machine are reported differently than on a PSG report. The in-lab machine (Alice) sees them just the same but the person scoring the data takes into account the arousal state of the patient.
All events that occur related to an arousal are not reported.

So … if, as suggested, the CA's you are showing on the OSCAR report is related to an arousal (and it seems so to me also) the in-lab study would not have reported it and, unless the time duration gets to be longer, they can be safely ignored.

However, when you see you sleep doctor next, do mention them to him and show him the same detail that you have posted - he will have his own opinion as to their significance.

Awake flagged events don't count.

Try turning EPR off and see if the number of centrals reduce or not....at least the number that can't be blamed on awake breathing.
