Ok, epr will try one less than full tonight.. what does less EPR or no EPR benefit in my situation?
If I only side sleep would having more EPR or less EPR matter then I never get CSR on my side. I've been using my APAP for 3 months. I couldn't wait for my sleep study. I was waking up with high blood pressure 180 over 120 and the emergency room people were just laughing at me telling me to go home and giving me a shot of Benadryl.
I also started testosterone therapy in July after going for 12 years with extremely low testosterone I was causing me to have hypoglycemic grandma seizures from metabolic and unflexibility. Maybe my CSR is from neuro damage.. but I do remember waking up with my ex-wife prior to any seizures having her over me on my back looking at me while I was suffocating so to speak.
I thought she was choking me.. lol ..
you guys all have a good night and I will be looking at this form in the morning again thank you so much

There's a very small percentage of people who use bilevel pressures that will sometimes develop central apneas.
Using EPR or Flex exhale relief essentially creates a bilevel pressure situation. We don't know that is a trigger for you but the easy way to find out is turn it off or reduce it. Easy cheap experiment.
As to why you only have a problem when you are on your back...beats me. Something about the way you end up breathing when you are on your back is causing the unstable breathing.
People can have idiopathic centrals and CSR which means no known cause and doesn't mean they are having any significant health issues or close to dying or anything like that.....but they still need to jump all the hurdles to make sure nothing bad is going on with the heart just to be on the safe side.

Thank you for your input and knowledge.. I believe my dad had some form of this also before his heart failure and I know as a child I could never fall asleep and or if I was awoken on my back I always was in a panic. well I am definitely taking care of myself and making sure that there is nothing else wrong with me and I really appreciate just knowing that there is such a thing as CSR in the non hf setting..
For those of you that have an ASV machine for centrals I'm wondering if just using it to try to sleep on my back would be worthwhile. My shoulders are sore. I can buy a used ResMed s9 ASV for $500 just to say I can sleep on my back maybe worth it if it wasn't any damaging effects.
I don't mind experimenting..

The nice thing about ASV....it only does the treating for the centrals when/if you have centrals.
So if you don't have any centrals then it will just deal with the obstructive stuff as it happens.

As long as the one you're looking at doesn't have a ton of hours on it, that's a pretty good price.

I found 2 units from the same guy .. s9 and a s10 .. I'm awaiting model numbers .. he said they have 2k hours .
Here is a small side note ..
My sleep study doctor who wrote my script said that my csr style breathing wasn't csr.. and that id need a special sleep study for ASV.. I figured he didn't even look at my flow limitations ...

You can see my my issues during back sleeping.. I am do for a partial knee surgery and afterwards I'll be off workman's comp .. then I'll have my normal insurance back ..
Then start this all over again with them sure with a new sleep study..

As far as I know, there's only one Aircurve 10 ASV model. (There's no "s10" anything from Resmed).

So I found and bought a resmed aircurve asv used from Craigslist for 700 bucks with 700 hours .
My question is with my style of CSR style breathing while on my back will the machine be constantly trying to correct my breathing or will it need just one puff or so to kick start me back to normal..
I kind of feel like it's going to be doing its thing every breath.. and if so am I going to be able to maintain my sleep while it's doing it..

The ASV will do what it needs to do.
If you find your awake and it seems to be blowing at you breath back hard agains the machine and it will back off for 3 minutes. An ASV can take a couple of night to get used to but most people end up will an AHI of less than 1 every night.
I suggest you start on
ASVAuto mode
EPAP 5
Min PS 3
Max PS 15
Ramp off
Ramp is bad for ASV as the machine needs to align with your breathing before you go to sleep which it can’t do if you use ramp.
If you find your awake and it seems to be blowing at you breath back hard agains the machine and it will back off for 3 minutes. An ASV can take a couple of night to get used to but most people end up will an AHI of less than 1 every night.

I will give it a shot and reply back soon ..I've watched some videos on YouTube also of a guy saying how to set up the device the same way .... it's amazing how people from a community forum like this are able to bring so much help. Compared to your average sleep doctor..

Your support network of doctors don't realize that this isn't like a prescription for a pill. It's like a prescription for an exercise program. It will take time and even a lot of time to get used to it and get the settings and gear right. Once you get it all working and comfortable you'll never want to sleep without it.
It took me nearly a year but I'm very sensitive to anything bugging me.

Ok got the asv set up to your recommendation..
Had some issues..
When I was just getting sleep onset I would pause my breathing and the asv blasts off with pressure and wakes me back up .. repeatedly..
So I taped out and went back on my apap and had no issues..
Is the ramp function the only way around this problem ?
I would breath normally while awake and I did notice it seems like it was topping off my peak inhale. Like a extra more than my natural depth.. why ?

I use a lower PS. 10 is good enough for me. If I recall, my therapy wasn't effective with a PS of 7-8, and a PS of 15 was a bit much. ASV takes some getting used to. I don't use the ramp at all.
