Sleep Woes-Need help with Oscar results

Hi everyone,my first post here. I was diagnosed with moderate to severe sleep apnea back in year 2000. Back then no way to read data from old machine. I was fitted with an Ultra Mirage II and used that model for a number of years. My initial pressure was 10 then increased to 11 after another sleep study in 2005. Stayed with that setup for some time. Early this year I complained about fatigue so did another sleep study this time at hospital. Results were given to increase pressure to 12 then thirteen after many appointments. Just could not control the leak value. My DME started a mask switch as as I was getting a dent in nose bridge that bled. I tried every mask from full face to nasal and pillows that they could give me. Just this past week I tried pillows which I could not wear previously. Through out this year my ahi was 25 to 35. This past week have been better since switch to pillows. I will gladly supply more information if needed. I am attaching Oscar graphs for your perusal. I am self medicating since next hospital visit is not till next year. I have increased pressure to 14.4 so far and shut off EPR.Much appreciate any help possible to get this under control.I should add that I have Afib, deviated septum and fibromyalgia plus a hiatal hernia.

New wrinkle now. My machine is rebooting thru the night. It woke me up last night every time it did that. Any ideas? I am attaching last nights data.

I think it is shutting itself off during the night because the leaks are so bad it doesn't know you are attached to it.

Turn SmartStart off and that won't happen but you have a bigger problem...the leaks.

Hello, cevetsky. Where do you think the leaks are coming from: your mouth, the nasal interface, or some of each?

Every nights data you posted shows numerous breaks in the flow and pressure trends but you say the machine turning off was new that last night? Were you shutting off the machine ~15 times the first 3 nights posted? What does your older data look like(last week, month, 3 months ago etc)? Do the graphs show similar breaks in data, getting worse etc?

Do you know what causes the leaks? If you have tried all the different types of masks is it due to pulling the mask off? Maybe unconsciously doing so because your machine shuts off on you? Just an idea.

I would try Pugsy's recommendation, turn smart start off and see if that helps with machine shutting off.

I woke up last night from the machine restarting but according to my wife this has been going on for quite sometime. I have never manually shut machine off. I think the breaks are getting worse but woke me up for the first time last night.

Smartstart turns the machine off if it believes you aren't wearing the mask. Turning it off will determine if that is what is causing the problem, based on the regularity of these events tomorrows data should be able to tell you if turning this setting off fixed this issue.

Well turning smart start off solved the breaks but I know the leak is horrendous. I am having a difficult time understanding this because this morning and as well in the past the mask fit from machine says the mask is ok. I guess I will play with settings and see if could help. Going back to hospital in February so might be time for another sleep study even though I had one this year.

My best guess...2 potential sources for the leaks.
Mask movement as you toss and turn during the night or mouth opening leaks letting air exit the mouth.

First thing I would do though is tape my lips shut so I could hopefully figure out if it is mouth opening that is the leaking cause or not.
To fix a problem you first have to identify the problem.
I would hate to go through a lot to time and expense with mouth opening fixes only to find out it was mask movement.

Taping is the most sure way of identifying if the leak is mouth or mask movement. If the tape is secure in the morning and no leaks...then we know there are mouth opening issues. If the tape is secure in the morning and no improvement in the leak situation then we know it is mask movement leaking.

Are you doing anything to try to help keep the mouth/lips shut?
If not...you need to try something.
If you are already then maybe need to re-evaluate what you are doing.

First off thanks for the response. In regard to taping I did try that a couple months ago for one week. I still got leaks. I now am switching between three different masks. I use a Pap Cap chin strap which I have used for years.If your conclusion is mask movement then it must happen with the three different masks. Do you believe that the pressure is too high? The hospital set it at 13 from previous 11. I have been toying with some settings to see if any improvement and upping pressure is one I set higher. I am frazzled.

I personally don't think that pressure causes mask movement leaking. I think poor fit and/or tossing and turning causes it.
I am betting if your pressure was 5 it would still show massive leak.
