Brand New and Have Questions

Post by Goldsmoke97 » Thu Oct 31, 2019 8:05 pm

Hello Everyone,
I started my cpap 10/22/19, I am 32 year old male a little overweight but nothing to serious. My machine setting is 5-15
After I had my initial sleep study I got a call from the doctor's and they asked me if I wanted to take a second sleep study, which would have been at least another month, or I could just pick up the machine and if they're any problems I can stop in. I opted to get the machine since I knew a few folks that had the and they loved it.
First two night I just was on my back and I didn't even feel like i fell asleep and the myair app pointed out that I had 7-9 event during those nights. Since then I started to sleep in a recliner, which helps but I still find myself waking every few hours. I've since visited my Dr and we're waiting on insurance to approve the second sleep study.
This is from my most recent night, I tried to set it up correctly and I'm hoping one of you vets can point me in the right direction.

Hello, and welcome to the forum. :D

You are going to need more minimum pressure, most likely.

In the future posts:

When you post your chart leave the left sidebar on - minus the calender and pie chart.

Set up the graphs :

Events

Flow rate

Pressure graph - not mask pressure

Leaks

Flow limit.

Keep future posts in this thread for now.

An expert will be along to guide you further. :)

Except, flip leak rate and flow rate.
I personally like them set up like this.
For my personal perusing.

You're right about Oscar being better, I just didn't want to go and re-do the 'how to' thing when I know that Oscar's going to change a number of the menus and some of the way things are displayed fairly soon...

Yes, lazy, I know :)

Congratulations on starting therapy for your apnea! You have an excellent machine, you seem to have amazing mastery of mask leaks, and you've come to a forum where there are people who can help you. Plus you're using the machine all night! In all these ways, you're off to a good start, and with some patience and adjustments, I think you are likely to start sleeping better at night.

Do you know what your AHI (apnea/hyponpnea index) was in your first sleep study? And do you know how many apnea events were obstructive and how many were not? Finally, do you know what your doctor hopes to find out with the second sleep study? Answers to those questions, plus a reformatted chart, might help the experts help you.

First I want to thank you all for replying the process started to get scary after the first few nights and coming to this forum has helped keep up with the therapy

Miss Emerita: my AHI was 29. something I actually didn't know any of my lab results prior to getting the machine and only found out this past Tuesday. At that time, and still, I have extreme fatigue I think I'll call them up today and see if I can get a copy so I can answer both of your question .

The second sleep study is to determine if my body is just doesn't react well to cpap and if a bi-bap machine might work better for me. My first night with the cpap, I was having an evident and the pressure rose to 14.2 and I was woken up by the air pushing through my throat. The Dr also seemed concerned that I wasn't able to sleep through the night with the machine

A little more about me, I normally sleep elevated and on my side so switching to my back for the full mask was a bit of change.

This past summer I had my gallbladder removed, since I was unable to sleep on my side I had to sleep on my back and I started waking up every 45 mins from sleep apnea. Ultimately it worked out but note before 4 days of the worst insomnia of my life. ( although the cpap is second and may catch up). and currently take an anti-anxiety/depression med.

The final thing: I feel ok when I first wake up but about 3 hours later I crash really hard and I'm not drinking any caffeine or other stimulants is this normal?

Good job on formatting the chart! Yes, it'll be helpful to get a copy of the sleep study. (You have a right to have it, by the way.) Get the whole study, not just the summary. You can post it here, first removing identifying information.

What I'm especially curious about is whether you had a lot of central apneas in the sleep study. (These are cessations of breathing where there is no obstruction of your airway.) If you did, your doctor may be thinking you'll need a more specialized machine and may be starting you on the process of trying and failing with less specialized machines. (In this case, you would probably not stop with a bilevel machine but go on to something even more specialized.) If you didn't have many CAs to speak of in the sleep study, then the ones we're seeing now may be "treatment emergent." They would probably just go away on their own over time. Wouldn't that be nice!

When I've had a rocky night, I feel bad from the minute I get out of bed, but I think people vary a lot. So the pattern you describe is may just be your way of registering what your night was like. The goal of therapy will be for you to get your AHI way down, for the sake of your long-term health, and to have you feeling all day the way you do when you first get up, so that your day-to-day life will be much better.

Thanks for the quick reply! Luckily I live close to my sleep doctor and was able to pick up a copy of the report, but I think it may be a summary. Thanks again for the words of enourgement I wish someone stopped me before the process and just told me how important it is to find a support network.

Tonight try either turning off EPR or at least reducing EPR significantly.

I did make some changes to my sleep practice, I moved from the recliner back to sleeping on my side in my bed. Looking through all of my daily charts I did notice that last night was my second highest "Total Time in apnea" at 21:31 with the highest being from my first night at 26:03. Is that bad?
