It does not appear that you're having significant central events. You seem to just have a very bad case of Obstructive Sleep Apnea that is not well controlled by CPAP.

I'm not sure if a BiPAP would help or not. However, a BiPAP can go above 20cmH2O.

Since you're new here, take everything DH says with a grain of salt. He's not *always* wrong, but finding the nuggets of truth is not for the faint of heart .

Thanks for your comments, D,H. The old auto cpap was set up for 4 min and 20 max. 95% of the time the pressure is at or below 11. Never has the pressure exceded 15. ResScan charts show constant apneas every 20-30 seconds and the graph showing the pressure never spikes up any appreciable amount, leading me to believe they are mostly clear airway apneas.

same night. 10 hour view. number of apneas was maxed out so the machine quit counting.

I bet 90% of your problems would go away if you raise the minimum (not max) pressure setting to 7 or 8... doctors (who understand apnea but not equipment) routinely prescribe the default low numbers thinking the machines can catch up to events in time to help no matter how high they are... and the machines can't, not to mention that most find 4 a very difficult setting to inhale at. So why not see how things go at a higher min. (apart from whatever PMs you get).

Julie, thanks for the suggestion. Several weeks ago I reset the min and max to 9 and 15 and tried a narrower range. I also tried loweing EPR from 3 to 2 for several days, then 2, 1, and 0. I also tried straight CAP at 11, and 4. Nothing seems to change or affect the AHI, which ranges from 40+/hr.

I will try temporarily using a more modern machine that is sleepyhead-compatible, next week. After collecting better info, particularly the separation of the number of obstructives and centrals via sleepy head, I will post new charts.

The S8 doesn't/can't tell the difference between centrals or obstructive.

Did you people saying centrals not a problem not see this part?

Did you miss these results?....pressure minimums of 9 to 11...look at the AHI with higher than 7 or 8...
Old data yes...minimum of 4 but maybe you need to take the time to look at all the data available.

I know that obstructives are problem....and maybe the centrals are also a problem but until he can use a machine that can tell central from obstructive...we are just guessing and you all know I don't like guessing.

If you are using Medicare to purchase your machine, you will do a 13 month rent to own program. Your portion of the rental will probably be around $20 a month for the 13 months. If you start with an auto bi-level machine, then need to switch to the ASV, you will start a new 13 month rent to own program for the new machine and your old program will be terminated.

Okie Bipap, that's good to know. I figured I'd have to buy the first machine and then the second machine. I wii have access to a loaner so I can collect SH data, then buy the one machine that will truly address/resolve these events.

I am slowly evolving from disgust/anger to hope ("having only positive expectations") that these will eventally be properly treated. :)

Yeah, get past the disgust/anger phase as quick as you can. The newer machines are *WAY* better than the one you've got. they're much smarter, they're quieter, they've got *tons* more data to help you fine tune things...

There's lots of hope.
