You mean I'm not an android like Commander Data of the Enterprise? You ruined my dreams now. haha. Last night was super good again, at lowest pressure. Although it started with two OA's. My O2 dropped to low 80's and my Contec SpO2 sensor alarm went off, waking me up. No telling how low it would drop if i didnt have that alarm. I woke up and advanced my dental appliance a half millimeter forward and then went back to sleep to see if that would help. I can't advance it much more or i start getting dry mouth and breathing thru my mouth all night long.

I'm thinking PS 4 is ideal for me. But the low pressure certainly doesnt work for me on other nights. My ENT said I was a candidate for that "Inspire" electronic device that keeps the tongue forward. But I don't want to have a damn implant if I don't need to. I'd rather have some minor surgeries to reduce the tissue. They have tongue based reduction surgery now that is an in-office procedure.

Are you wearing the dental device every single night...and were you wearing it on the nights where the reports show your machine pegged out at the max of 15?

And yes...since the recent (limited sleep time) sleep study was done with the oral device in use...probably means that with the oral device and short hours of sleep those lower settings did work just fine.

Any mention as to REM stage sleep on any of your sleep studies? During REM is where I need the big pressure swings...non REM the machine barely ever moves in terms of pressure response.

With just a couple of hours of sleep it is unlikely that you got much REM even if you did stay in one position all night long.

Yes, I've been wearing the dental appliance every night. And wore it on those nights where it pegged out at 15. According to my last sleep study, I was totally asleep for about 5hr. It took me 10min to fall asleep. It took 167min to go into REM. stage N1 9.5%, Stage N2 52%, Stage N3 32%, 5.8% REM sleep.

My older sleep study back in Febuary, when they titrated me at 16/12, with most appropriate setting 14/10, said I had ZERO REM.. This was before my dental appliance.

Pugsy's pressure needs range from 7 to 18 *in a single night*... so much for that "we only need two hours to get a good number" BS.
I got a call earlier from my doctor office. I told them I'm considering leaving for another sleep doctor. I then got a return call. The nurse said one of the sleep doctor's there said we could try a FIXED pressure of 14/10 PS 4. This is the setting they titrated me at during my sleep study in February. She said that 14 was always the highest I had. I said, yes...that is the highest because the machine was pegging out at 14 and it was locked down to not go higher. She said they didn't want to set it at a very-wide range since that is not beneficial. I then asked, what about starting my pressure a little higher and going higher then 14 then. I told her it pegged out at 15 and even 16 when I had it set there. She seemed to not have any more ideas and said to just let her know what I want to do, either set it to a FIXED pressure 14/10, or otherwise. I ended the call and told them I'd think about.

Personally, I don't want to go TOO high, because that caused CA's, and also caused me to wake up with extreme anxiety. Like I was getting too much air all night long. So it's a double-edged sword for me. Do you think I should have them set it to FIXED pressure 14/10 PS4? Or have them raise both the min and max?

Right now it is set to start IPAP 8, EPAP 4, PS4, Maximum IPAP 14. If they change it to up the pressure at 14/10, this could cause my AirTouch F20 to leak more. This is considerably more pressure then I need most of the night. Usually lately 8/4 worked for several nights now in a row, and it never went higher last night past 9/5.

Last night... Excellent at lowest pressure. Except for 2 OA's at beginning that caused my SpO2 to dip to low 80's.

All I know is I do not roll on my side or anything. Four sleep tests, that each told me I never moved from the supine position. I did kick my legs since i have restless legs, but that was it.


Anyway, last night was a GREAT night, at LOW PRESSURE! IPAP 8, EPAP 4, PS 4. I got AHI 0.0 for one of the first times.I guess that sort of shows that LOW pressures do work better for me most of the time. It's only on occasional nights, I need higher pressures. The only thing I notice is that every 20-40min, I'm taking a deep breath for some reason. But I've always done that. Probably a learnt response to apnea or something.

Can you take this screen shot and zoom in to 120 and 300 seconds? Perhaps those OAs are not OAs.
